Many physical therapists start their careers excited to help people and make a difference, but it doesn't take long before the reality sets in. Long days, unfinished notes, and documentation that follows them home at night can quickly lead to burnout. At our clinic, we believe it shouldn't be that way. You shouldn't have to choose between being a great clinician and enjoying your life outside of work. That's why we've intentionally built a different kind of environment, one centered around true work-life balance and long-term sustainability. When you take time off, you're able to fully disconnect. You're not responsible for finding your own coverage, we handle that as a team so you can step away without stress or guilt. During the workday, your schedule allows for natural time throughout the day to stay caught up on documentation. You're not expected to take notes home, and in fact, several of our physical therapists haven't taken documentation home in over two years. That's not by chance, it's by design. With that built-in flow of time and the support of an AI-assisted scribe, you can finish your notes during your workday, so when you leave, you're done. You can be fully present at home, with your family, and in the parts of life that matter most.
Reasonable Productivity Expectations
Many outpatient clinics expect Physical Therapists to see 65 or more patient visits per week, often leaving clinicians rushing through treatments and taking documentation home at night just to keep up. We believe great therapists shouldn't have to sacrifice their evenings, weekends, or personal time to meet unrealistic productivity expectations. That's why our clinic takes a more balanced approach. At Madden and Gilbert Physical Therapy, our expectation is around 60 visits per week, giving clinicians the time they need to provide thoughtful, high-quality care, complete documentation during the workday, and apply strong clinical reasoning for each patient. In addition, our Clinical Director meets individually with each Physical Therapist to review a personalized scorecard, providing clear feedback, support, and direction for continued growth. This ensures that expectations are transparent and that every clinician has guidance on how to succeed, not just in productivity, but in delivering meaningful patient outcomes. By keeping productivity reasonable and communication clear, our therapists can focus on what matters most: helping patients get better while maintaining a sustainable and rewarding career.
Structured Mentorship Built Into Your Schedule
Many new Physical Therapists enter their first job excited to learn, only to find themselves quickly overwhelmed and expected to figure things out on their own. We believe new clinicians deserve real guidance and support, not to be left alone to manage complex cases. That's why mentorship is built directly into how we practice at Madden and Gilbert Physical Therapy. Every therapist receives a protected 40-minute one-on-one mentorship meeting each week with our Clinical Director, who has over 12 years of experience and is also a partner in the practice. In addition, new clinicians work side-by-side with the Clinical Director for at least six months, allowing for real-time collaboration and hands-on guidance to build confidence and strengthen clinical decision-making early in their careers.
Certified Orthopedic Manual Therapist (COMT) Training
Many physical therapists graduate excited to grow, but quickly realize that advanced training is often expensive, difficult to access, or unsupported in their workplace. Instead of progressing their skills, they can feel stuck repeating the same treatments and lacking confidence in their clinical decisions. At Madden & Gilbert Physical Therapy, we believe you shouldn't have to choose between working and continuing to grow. That's why our mentorship program includes our in-house Certified Orthopedic Manual Therapist (COMT) training, an investment of over $3,000, at no cost to you. This program takes you from the fundamentals of orthopedic evaluation through advanced manual therapy techniques, helping you develop stronger diagnostic skills, hands-on treatment ability, and true clinical confidence. You'll learn not just what to do, but why you're doing it, allowing you to think critically, adapt your approach, and consistently deliver better patient outcomes. The result is a clinician with stronger manual skills, clearer clinical reasoning, and the confidence to continue growing throughout their career. Our Treatment Philosophy
Many physical therapists find themselves in clinics where patients are frequently passed between providers, making it difficult to build meaningful relationships and truly guide a patient's recovery. Over time, this can lead to frustration, limited progress, and a disconnect from the impact you set out to make as a clinician. At our clinic, we believe there's a better way. Continuity of care leads to better outcomes, not just for patients, but for clinicians as well. That's why at Madden & Gilbert Physical Therapy, the therapist who performs the evaluation continues treating that patient throughout their entire plan of care, from the first visit to discharge. This allows you to build strong relationships, communicate more effectively, and take ownership of each patient's progress and outcomes. We also equip our clinicians with advanced tools to support that care, and these tools are integrated directly into your clinical decision-making and treatment plans. This includes the use of a Class IV laser to help reduce pain and inflammation early in care, allowing patients to move better and progress more quickly into active treatment. Our hyperbaric oxygen therapy unit provides an additional option for patients who need enhanced healing support, particularly in more complex or slower-to-respond cases. For our running population, RunDNA technology allows you to analyze gait mechanics, loading patterns, and movement inefficiencies in detail. This gives you objective data to build highly individualized treatment plans, moving beyond general exercise prescription to targeted, performance-driven care.
By combining continuity of care with advanced treatment options and strong clinical reasoning, you're able to deliver more personalized, effective care while continuing to grow your skill set. The result is a more engaged clinician, stronger patient relationships, and better overall outcomes.

We believe every clinician should have the support to grow in the direction that fits their goals, whether that means becoming a leader in the organization or simply continuing to develop as an excellent clinician. At Madden and Gilbert Physical Therapy, we develop leaders from within and offer a clear growth pathway from Staff Physical Therapist to Team Leader, Clinical Director, and eventually Equity Partner. At the same time, we fully support therapists who prefer to focus on clinical mastery without pursuing leadership roles. Several of our current Clinical Directors began their careers as Staff Physical Therapists and advanced through this exact pathway.
