SUMMARY DESCRIPTION

The primary focus of this position is to be responsible for the establishment and maintenance of a comprehensive Caregiver Resource Center in the CHEER Community Center, as well as satellite locations in other CHEER facilities. The Caregiver Resource Center provides trusted and meaningful information and assistance/referral, community outreach, education, and access to services and assistive technologies that support caregivers. It is a part of the caregiver action network and is a hub for resources, activities, and support.

ESSENTIAL DUTIES & RESPONSIBILITIES:

This listing is representative of the duties and responsibilities of

the position, but is not to be construed as all inclusive:

Research, maintain, disseminate, and track the distribution of

resource materials pertaining to caregiver issues, assistive technologies, demographics, and support services available for caregivers.

Assist in identifying, assessing, and updating resources for

caregivers provided by the Delaware ADRC searchable database, Guide to services for older Delawareans and adults with disabilities, and related directories and online database

Remain informed and provide resources on current and emerging

caregiver issues

Be available at the Caregiver Resource Center during designated

staffing hours.

Support the development of new and existing support groups that

focus on caregiver issues.

Serve as a liaison between the Caregiver Resource Center and the

Division of Services for Aging and Adults with Physical Disabilities.

Conduct initial counseling sessions/interviews of individual

caregivers and clients either in-person or virtually, per the preference of the caregiver.

Maintain and expand the number of caregivers attending support

groups, trainings, one-on-ones, and conferences/expos, as defined by OAAPS.

Respond promptly to all contacts via phone or email. Answer and

return phone calls, make follow-up calls, research, and respond to specific caregiver issues and concerns.

Provide person-centered information, assistance, and direct access

to support services and resources to caregivers as needed. Refer to appropriate agencies or organizations that support caregivers.

Refer to intervention activities, in-person or virtual, that support

caregivers, including: Special support groups that focus on coping skills and reducing the risk of caregiver burnout. Evidence-based, self-management programs for caregivers

Participate in community outreach health fairs, expos and related

community activities regarding caregiver interests. Conduct community outreach at a minimum of two (2) events per year.

The CRC will conduct and facilitate in-person or virtual caregiver

training and/or support groups at a minimum of two (2) per month

Host a calendar of in-person and/or virtual workshops and/or

seminars on relevant topics for caregivers in the community at a minimum of two (2) per quarter.

Facilitate and participate in a minimum of two (2) in-person

appointments per month that promote the Caregiver Resource Center, support services and related resources to local public and private community or faith-based organizations, physician offices, pharmacies, clinics, senior centers, and other stakeholders at a minimum of three (3) hours per month.

Record, track, and submit all data inthe DSAAPD Invoicing Workbook

Caregiver Resource Center.

Prepare and submit required Monthly Reports.

Participate in caregiver support group activities i.e. Parkinson

Caregiver, Alzheimer Caregiver & Memory Cafe.

Conduct promotional activities throughout the service community to

increase awareness of the Caregiver Resource Center and the services it provides.

Participate in formal training or cross-training activities that

will further professional development and understanding of caregiver needs and services at a minimum of two (2) per year.

Perform other duties as may be deemed necessary by the Deputy

Director for the safe, efficient and effective operation of the Community Center and its associated programs and services.

PRIMARY WORKLOCATION:Most duties and functions will be performed in an office environment, although local travel to other CHEER locations is required to support programs and services as required in those locations.
