Responsible for providing continuous observation and appropriate documentation on activity of an assigned patient who has been identified as having increased risk for injury.

Principal Accountabilities:

Provides ADL's and protects Patients in Sitter and Spotter assignments from self-harm and prevent elopements.

Makes continuous rounds.

Assists with Activities of Daily Living.

Assists with bathroom/ commode/incontinence care.

Monitors Intake and Output.

Performs preventive skin care, feeding mouth care, and changes bed linens when necessary.

Performs other related duties as assigned.

Required:

High School Diploma or equivalent.

Prior experience in Patient Care.
