Job Title: Planning List Navigator- Region 6 Field Office- Columbus

Pay Grade: I

Workplace: Hybrid A- 3 to 5 days per week in office

Travel: less than 25%

Job Summary:

The candidate chosen for this position will perform the duties listed below in multiple counties in West Georgia. The position is located at the Region 6 Field Office in Columbus, Georgia.

Must be a highly organized individual, as this position will require a significant amount of tracking and follow-up for many different job functions Requires completion of a significant number of Needs Assessments for new individuals as well as annual renewals for individuals on the Planning List Requires telephone support to individuals and families on the Planning List, and bringing resolution to phone calls and messages Requires reporting Planning List changes and concerns to Planning List Supervisor Make and resolve referrals to outside agencies for non-waiver services Other supports for Planning List as needed.

Preferred Qualifications

Experience with or knowledge of individuals with intellectual/ developmental disabilities, Medicaid waivers, and DBHDD I/DD services; experience working successfully without daily contact with a supervisor. Spanish fluency

This position is located in the DBHDD Region 6 Field Office in Columbus, Georgia. DBHDD Region 6 service individual residing in the following counties: Butts, Carroll, Chattahoochee, Clay, Coweta, Crawford, Crisp, Dooly, Fayette, Harris, Heard, Henry, Houston, Lamar, Macon, Marion, Meriwether, Muscogee, Peach, Pike, Quitman, Randolph, Schley, Spalding, Stewart, Sumter, Talbot, Taylor, Troup, Upson, and Webster.

Employment Requirements

Bachelor's degree in a related field from an accredited college or university and two (2) years of job-related experience; or two (2) years of experience at the lower level Social Svcs Prgm Coord 1 (SSP030) or position equivalent. Note: Some positions require eligibility for licensure.
