Summary The Hemodialysis Registered Nurse (RN) is a specialized professional who provides and improves care for patients with end-stage renal disease, acute kidney injury, and late-stage chronic kidney disease. This RN acts as a primary nurse for ESRD patients & help patients navigate the healthcare system. They must demonstrate leadership & fostering a safe, supportive environment, they contribute to discussions with interdisciplinary teams, support colleagues, and promote evidence-based practices. Responsibilities Major duties include but are not limited to: Ensures proper documentation, resolve patient care problems, and direct nursing education and quality improvement initiatives. Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Completes documentation for all clinical and technical recordkeeping in the electronic health record and treatment flowsheets in accordance with established policies and procedures. Improves Veteran/patient driven care by resolving experience issues such as coordination of care, timeliness or process issues, common themes of dissatisfaction, or environment of care concerns for the immediate practice setting. Participates in the advancement of the profession through mentoring, precepting, and/or teaching. Knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program.
