In this role, you will deliver high-quality nursing care and coordination for Veterans in the Home Health Program. Key duties include: Conduct home visits to provide direct, technically competent nursing care for Veterans with complex health needs Perform comprehensive assessments and develop, implement, and evaluate individualized, age-specific plans of care. Coordinate care with a strong focus on patient education, self-management support, and overall Veteran satisfaction throughout the continuum of care. Administer prescribed medications, treatments, and procedures in alignment with established policies, guidelines, and best practices. Promote health maintenance and prevention of illness and injury through systematic assessment, health teaching, counseling, and restorative nursing interventions. Deliver supportive and restorative care while executing prescribed medical regimens to enhance quality of life and well-being. Perform triage, coordinate medications and treatments, and provide ongoing Veteran and family education within the Home Health Program.
