Summary The GS 12 Supervisory Social Worker is responsible for direct supervision, Clinical coordination and Leadership of Patient Aligned Care Team (PACT) Social Workers assigned to the Primary Care Division at the Clement J. Zablocki Medical Center in Milwaukee, WI. The PACT is a comprehensive team which delivers primary care to Veteran patients in a longitudinal rather than episodic fashion and which has as its focus: prevention; health promotion, coordination and chronic disease management. Responsibilities Major Functions of the PACT Supervisory Social Worker include: Administers information and analytical systems to evaluate and enhance the quality of service provided to patients. Develops and implements information systems to track service activities including visits, referrals, patient demographics, and patient satisfaction. Develops, implements, and evaluates clinical guidelines and protocols to establish appropriate utilization of services. Provides direct supervision and coordinates the day-to-day activities of a group of Social Workers performing work at GS-11 or GS-9 target GS-11 level in Primary Care clinics. Provides clinical supervision, administrative support, technical assistance, and other guidance as needed to accomplish the work and promote successful outcomes. Establishes guidelines and performance expectations for staff, which are then communicated through the formal employee performance management system. Observes workers' performance Provides informal constructive feedback, demonstrates and conducts work performance critiques, and completes the formal employee performance appraisal process. Resolves informal complaints and grievances through appropriate channels. Provides advice and counsel to workers related to work and administrative matters. Recommends disciplinary measures and takes appropriate administrative actions within the scope of delegated authority. Reviews and approves or disapproves leave requests. Assures that subordinates are adequately trained, meet clinical competency requirements, and fully comply with the provisions of safety regulations. Coordinates and shifts employee workload to accomplish the work of the Primary Care Clinics. The Social Work Supervisor is responsible for work assignments and review and supports the Program Manager and Division Manager with other administrative and personnel management functions relative to the social work staff assigned to Primary Care. English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j. Education: Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: You must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. This is typically satisfied at the LCSW level and varies per state requirements. GS-12 ASWB Examination: Complete and pass the Association of Social Work Boards (ASWB) advanced generalist or clinical examination. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). GS-12 Level: You must possess one year of experience equivalent to the GS-11 grade level (VA or Non VA) Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment In addition to the experience above, you must demonstrate the following Knowledge, Skills and Abilities: (a)Ability to independently organize work, set priorities, and meet multiple deadlines. (b) Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement.
