Description The Care Management Coordinator provides essential clerical support for the Care Management Department. Under the supervision of licensed clinical staff, accountabilities include coordinating communication between patients, health care providers, insurance companies and other stakeholders to ensure safe and efficient transitions of care, payment for clinical services provided and regulatory compliance. The goal of the Care Management Coordinator is to ensure the maintenance of accurate records, process documentation and support the delivery of quality patient care. Required Qualification: 2 years of administrative experience, healthcare setting preferred or demonstrated knowledge of healthcare terminology. Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
