Summary The Primary Purpose of the Position is: to provide care coordination and psychosocial services to Veterans who utilize the Community Care program. Responsibilities The Primary Purpose of the Position is: to provide care coordination and psychosocial services to Veterans who utilize the Community Care program. Screening/Assessment/Treatment Planning/Goal Setting: The incumbent must use a high level of skill in assessing and treating the complicated psychosocial problems of Veterans and their families/significant others. Develops psychosocial treatment plans in coordination with interdisciplinary team members. Social work responsibilities provide crisis management and develops psychosocial and clinical assessment of psychosocial problems that cause distress, often impacting the health condition and creating barriers to care. These stressors can include adjustment to the current medical condition, untreated or under-treated mental health or substance abuse condition, economic instability, legal problems, and inadequate housing and transportation. Administrative Functions: Is responsible for supporting the mission, policies, and procedures of the Department of Veterans Affairs, the Veterans Health Administration (VHA), the appropriate Veterans Integrated Service Network (VISN), and the facility. The Social Worker is willing to serve on committees, work groups, and task forces at the facility as deemed appropriate by the supervisor and Chief of Social Work Services. Case Management/ Goal Setting/Discharge Planning: Provides case management services to Veterans serving as the liaison to other treatment programs. Social workers will engage Veterans and, when appropriate, other members of the Veterans support system, in an ongoing information-gathering and decision-making process to help Veterans identify their goals, strengths and challenges. Resources and Referrals: The Social Worker will serve as the subject matter expert on VA and/or community resources. The Social Worker will collaborate with other service providers in reassessing the Veteran's needs for non-institutional, institutional services/programs and entitlements. The Social Worker is responsible for educating the Veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the veteran's preference or that of his surrogate decision-maker. Advocacy: The Social Worker will act as an advocate when it serves the best interest of the veteran/family. Crisis Intervention: The Social Worker is experienced in making rapid assessments and developing crisis management plans to maintain patient in the home, for admission to acute, short term and/or long-term placements. Patient Education/Whole Health Coaching: The Social Worker provide education related to VA and community resources, entitlements, Advance Directives/Living Will and will refer veterans/families to the appropriate interdisciplinary team member for identified health education needs to include Complimentary and Integrative Health Services. The Social Worker will conduct health education and coaching services with training, support and materials from the Veteran's Health Administration (VHA) National Office of Patient Centered Care and Cultural Transformation (OPCC&CT). Education/Training/Supervision: Participates in the orientation of new Social Worker staff and other interdisciplinary team members. Coordination of Non-Institutional and Institutional Services: The Community Care Social Worker is responsible for the coordination of the referrals of non-institutional services such as but not limited to HBPC, ADHC, home hospice, skilled and non-skilled homecare services, substance abuse or behavioral health programs. Individual/Group Counseling: The Community Care Social Worker is able to independently work with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills if licensed as a Licensed Clinical Social Worker (LCSW). Supportive counseling/education: The Social Worker will provide Veterans and their caregivers with ongoing supportive counseling and education. Supervisory Controls: Incumbent reports to the Social Work Supervisor and Chief of Social Work Service. Practices independently within the scope of their licensure, using professional judgment and expertise to make decisions about services and treatment provided to Veterans and their significant others/family members. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. VHA may waive the licensure or certification requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure/certification examinations. In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting.
