Petitioner, Frank Resnek, is seeking to fill one position for a Personal
Care Aide at residence located at 6 Newbury St., Boston, MA 02116. This
is a temporary, full time one time occurrence position for employment
dates of April 1, 2026 to September 1, 2028. Job duties will include the
following: Administer bedside or personal care, such as ambulation or
personal hygiene. Report changes in employer condition to agent. Assist
with the administration of medication. Assist employer with household
cleanliness, hygiene, personal grooming, and nutrition. Provide personal
care and assist with daily schedule. Perform light housekeeping duties
such as cooking, cleaning and washing clothes or dishes. Assist with
planning, purchasing and preparation of meals. Check employer vitals and
blood pressure. Transport client to locations outside the home, such as
to physicians\' offices or on outings, using a motor vehicle, may need
to travel with client out of state as needed. 3 months experience
required in personal aide or health aide or related experience. This
position will not supervise workers. No other education, training, or
special requirements other than above are necessary. Transportation:
From the place from which the worker has come to work, whether in the
U.S. or abroad, to the place of employment, if the worker completes 50%
of the work contract period, the employer will provide advance payment
for transportation and subsistence (including meals and, to the extent
necessary, lodging) from the place of recruitment to the place of
employment. Upon completion of the work contract or where the worker is
dismissed earlier, employer will provide or pay for worker\'s reasonable
costs of return transportation and subsistence back home or to the place
the worker originally departed to work, except where the worker will not
return due to subsequent employment with another employer. The amount of
transportation payment or reimbursement will be equal to the most
economical and reasonable common carrier for the distances involved.
Daily subsistence will be provided at a rate of \$16.28 per day during
travel to a maximum of \$68.00 per day with receipts. Three-fourths
guarantee: The employer guarantees to offer work for hours equal to at
least three fourths of the workdays in each 12-week period of the total
employment period. Tools, equipment, and supplies: The employer will
provide workers all tools, supplies and equipment to perform the job at
no charge. On-the-Job Training Available: Workers will be provided with
on-the-job training to perform the duties assigned. Travel Requirement:
Area(s) of travel required may vary but will include to doctor\'s
office, grocery stores and to preform other errands as needed; The
frequency of the travel required will vary - The nature of the travel -
the employer is an elderly gentlemen that is in very poor health but
still enjoys outings including visiting family and vacationing in
Florida. Travel will be contingent on employer\'s health during the time
the worker will be with him. Miscellaneous: The employer will use a
single workweek as its standard for computing wages due. The pay period
is every two weeks. The employer will make all payroll deductions
required by law and will not make any deductions which are not required
by law. The employer will reimburse an H-2B worker in the first work
week for all visa, visa processing, border crossing, and other related
fees, including those mandated by the government, incurred by the H-2B
worker. Employer will provide this worker with housing at the employer
residence, free and clear. Employer will not be deducting housing from
worker paycheck. This housing and live in requirement is not optional.
The Employer hereby notifies the Department that neither employer or its
agent/attorney have entered into any agent or recruiter of H-2B workers.
Therefore, no copies of such agreements exist. Further the employer does
not currently plan to utilize any agent
