As a Physical Therapist (PT), you will evaluate residents’ physical abilities and develop individualized treatment plans to help them regain or maintain mobility, function, and independence. You’ll provide hands-on therapy, educate patients and families, and collaborate with interdisciplinary teams to achieve optimal outcomes.

Conduct comprehensive assessments of residents’ physical function and mobility using standardized tools. · Provide therapeutic interventions including exercise programs, gait training, and use of assistive devices. · Educate residents, families, and staff on safe mobility techniques and home exercise programs. · Document evaluations, progress notes, and discharge summaries accurately and timely in accordance with regulatory and company standards. · Collaborate with occupational therapists, speech-language pathologists, nurses, and other team members to deliver coordinated care. · Current licensure as a Physical Therapist (PT) in the state of practice. · Graduation from an accredited Physical Therapy program. · Previous experience in long-term care, skilled nursing, or rehabilitation preferred. · Compassion and commitment to improving residents’ quality of life.
