Hourly Pay: $120 - $135 / per visit

What You'll Do:

Conduct comprehensive patient evaluations and reassessments

Develop and implement individualized therapy plans in collaboration with physicians and care teams

Provide hands-on physical therapy treatments and monitor patient progress

Educate patients and caregivers on therapeutic exercises and equipment use

Supervise Physical Therapy Assistants and participate in interdisciplinary case conferences

Maintain accurate, timely documentation in accordance with care plans and policies

What We're Looking For:

Licensed Physical Therapist in the state of practice

Graduate of an accredited Physical Therapy program

Minimum 1 year of experience preferred (acute care, outpatient, home health, or community health)

Current CPR certification and up-to-date PPD or chest x-ray

Strong communication skills and computer proficiency

Compassionate, collaborative, and committed to patient-centered care
