POSITION SUMMARY The Community LPN promotes the physical and mental health of individuals by applying nursing processes to individuals in an ISL setting. Services are provided based on the unique needs of each individual, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing. Additionally, hours of service for Professional Assessment and Monitoring Services (PAMS) may be authorized based upon the health requirements of individuals supported.

ESSENTIAL DUTIES AND RESPONSIBILITIES As a Community Licensed Practical Nurse, you will:

Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care as evidenced by documentation and observation of positive patient care outcomes.

Work with the Community RN to provide individual specific training (nursing delegations) to direct support staff working in various ISL programs.

Perform independent nursing functions using established procedures, policies, guidelines and standards.

Provide Level 1 Medication Administration training to direct support staff.

Maintain communication with other LPNs, Lead Support Professional, Assistant Lead Support Professional,Professional Manager, Community RN, and Program Director.

Attend monthly meetings for ISL programs as needed.

Documents services in accordance with agency policy and requirements associated with Medicaid an

Department of Mental Health Certification and Contractual compliance.

Assist all individuals in our care with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.

Provide quality assurance checks to various programs to ensure quality medical care has been provided.

Assist Community RN in responding to all CIMOR, HRST and APTS referrals in a timely fashion.

Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated

Participates in on-call rotation with other nursing staff.

Conducts assessment of individuals within 7 days of discharge from hospitalization or ER visit, including skin assessment.

Develops plans to ensure that the medical and health care needs of individuals are met. This responsibility includes:

Ensuring appropriate medical follow-up.

Providing nursing delegations related to medical diagnosis, allergies, medication, diet and special emergency plans related to ongoing medical conditions.

Coordinating and reviewing lab work.

Coordinating care with clinics, outside physicians and hospitals.

Health and nutrition review and teaching.

Medication Administration and other Stafftraining as assigned.

Follows up on health-related concerns as identified by the Department of Mental Health RN.

Present a positive image and attitude when representing CVA/CHS

QUALIFICATIONS REQUIRED EDUCATION and EXPERIENCE

Graduate of an accredited school of Nursing, with valid registration as an LPN in the state of Missouri, or a compact licensure state.

Professional Certification in CPR
