Summary The Dayton VA Medical Center is seeking a Licensed Practical Nurse (LPN) to join our Primary Care Clinical Lab team in Middletown, Ohio. The LPN will participate and provide input in clinic process improvement and system redesign activities. Also performs phlebotomy duties using collection equipment and correct order of draw for each test ordered. This role will provide a full range of practical nursing care to patents with a variety of physical and/or behavioral problems Responsibilities Major duties include but not limited to: Provides a full range of practical nursing care to patents with a variety of physical and/or behavioral problems. Preforms assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the veteran. LPNs with designated lab duties will receive training and have competencies verified per lab/Ancillary Testing Coordinator. LPN designated to lab will performs phlebotomy duties using collection equipment and correct order of draw for each test ordered as outlined in DVAMC Laboratory Procedure manual. Completes clinical reminders that are due and appropriately documents patient care provided and assists in completion of forms for patients. Delivers nursing care according to the principles of Patient Centered Care/RSC and PACT. Recognizes critical patient/emergency care situations and initiates effective actions and summon appropriate help. Maintains current BLS and initiates and performs when indicated. Demonstrates knowledge and/or required safety skills in areas of assignment and, when appropriate, in the following: emergency preparedness plan, hazardous materials, fire plan, equipment failure emergency plans, and utility outage response procedures for their areas. Assists in teaching Diabetic, hypertension and self-care skills to the patient, family, and significant others. Assists with diagnostic or therapeutic procedures. Knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions, using judgment in selecting the appropriate order and sequence of procedures and treatments, and accurately recognizing, reporting, and recording relevant patient information. Ability to observe, identify, and response to a patient's needs for care including medication, equipment-assisted care, and patient/family education. Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies/procedures. Observes for, documents, and reports physical and/or emotional changes in the patient's condition from prescribed medications/treatments. Knowledge and ability to recognize urgent or emergent patient care situations and initiating appropriate emergency interventions as directed. Knowledge and understanding of human behavior, patient motivations and reactions to situations and the ability to appropriately utilize this knowledge in working effectively with patients, family members and other staff. Knowledge and skill in performing support duties for complex diagnostic tests and/or specialized practices or procedures, which includes preparing the patient and assisting in the diagnostic examination, preparing, and handling specialized instruments or other specialized equipment, and monitoring the patient's condition before, during, and following the procedure. Physical Requirements: The Dayton VA Medical center is a major interdisciplinary teaching facility, serving medical, surgical, inpatients and outpatients. There is a large outpatient psychiatric population, with admissions for medical co-morbidities. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continues walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required.
