POSITION HIGHLIGHTS:
Focus on improving systems and processes that enhance patient safety across the continuum.
Play a key role in the review, analysis, implementation, and improvement of patient safety.
Have clinical expertise to support select committees and taskforce workgroups as well as synthesizes patient safety event(s) information and determine appropriate follow up.
Work with administrators, managers, faculty and staff to develop improvements to prevent reoccurrence of patient safety events and to improve quality of care.For significant events with harm and other reportable events, this position will ensure that action items are completed as required and documented appropriately.
Close collaboration with the UWMC Patient Safety Officer and Risk Management to manage patient safety events and outcomes.
Will lead and/or facilitate patient safety projects, workgroups and/or committees and ensure that they are aligned with organizational goals and regulatory reporting requirements.
RESPONSIBILITIES:
Responsible for independent review and analysis of patient safety events requiring knowledge of state and national requirements for reporting and follow up.
Coordinate, synthesize and document information about patient safety events in preparation for the review process. Includes review of the online reporting tool Safety Net, of the medical records, private consultation with providers and staff and site visits to the unit(s)/clinics involved as appropriate.
Lead/facilitate Patient Safety Event Reviews, Root Cause Analyses and Quality Improvement Debriefs as required. Ensure that all meeting participants are briefed on QI protections and practices and facilitate proposed action items and follow up. Prepare appropriate documentation for publicly reportable events including Department of Health (DOH) and/or The Joint Commission (TJC) incident reports. Submit required documentation within appropriate timeframes and deadlines. Serve as subject matter expert for select events during DOH/TJC onsite investigations. Coordinate response to external quality of care concerns.
Manage and facilitate Patient Safety workgroups and committees as assigned.Promote and foster a culture that encourages reporting, peer review, and changing systems and processes to prevent medical errors.Lead committees and/or represents the CCE as appropriate.
Collaborate with Risk Management, nurse managers, providers, and administrators to finalize action plans based on resources required, potential effectiveness of interventions and other priorities. Assist with the delegation of responsibility collecting measurement data, auditing and follow up.
Analyze trends and provide actionable data to clinical leaders for the improvement of Patient Safety Outcomes. Serve as a liaison, consultant, and subject matter expert with regard to TJC National Patient Safety Goals, Sentinel Event Alerts and other regulatory requirements. Assist in TJC preparation and serves as an internal consultant for specific TJC Standards.Analyze NPSG data and make recommendations as appropriate. Coach others in data gathering and implementation.
REQUIREMENTS:
Current license to practice as a registered nurse in the State of Washington
Minimum of Bachelor's Degree in Nursing
At least three years of experience in a clinical setting
At least two years of experience in quality improvement
Previous leadership/management experience, prior experience with the standards and requirements of TJC and other regulatory requirements as they apply to hospitals, and project management experience strongly preferred
