JOB REQUIREMENTS: CLTS CASE MANAGER CLTS Case Manager CLTS CASE MANAGER The Washington County Human Service Department, located in West Bend, Wisconsin, has an excellent opportunity as a full-time CLTS Case Manager. The Washington County Human Services Department offers responsive and effective community programs to individuals and families challenged by disability, economic hardship, and safety concerns. Under the general direction of the Youth Treatment Team Supervisor, the Case Manager is responsible for the planning, delivery, and coordination of case management services for children with disabilities and their families. Qualified candidates must possess: Bachelor\'s Degree in a health or human services related field. A minimum of two years of professional work experience preferably working with children with disabilities and their families in community programs. Experience in waiver programs preferred. Must possess a valid Wisconsin driver\'s license. Starting hourly rate is \$28.21- \$37.50 based on experience.
