Summary Miami VA Healthcare System is looking for a Senior Social Worker-Homeless Outreach Program as the Contracted Emergency Residential Shelter (CERS) Liaison for their Flagler VA Clinic located at 1492 West Flagler Street, Miami, FL 33135. Senior Social Workers have experience that demonstrates possession of advanced practice skills and judgment. The Senior Social Worker operates independently in the field within the contracted emergency residential shelter settings. Responsibilities Total Rewards of a Allied Health Professional Senior Social Worker Contracted Emergency Residential Shelter (CERS) liaison is responsible for processing billing, providing administrative oversight, and offering clinical consultation. Additionally, the senior social worker provides direct services to Veterans, facilitates community meetings, performs clinical case management, and collaborates with the contracted provider's case manager to establish permanent housing stability plans. The CERS liaison senior social worker also participates in process improvement activities related to the administration of the CERS program. The role is dedicated to serving a complex population that may have severe substance use disorders, mental health issues, and/or physical health challenges. Senior social worker must display a high-level of professionalism and organizational stewardship in all dealings with the community as a representative of the Department of Veterans Affairs. The CERS liaison senior social worker is a critical position within the Miami VA Healthcare System and serves under the direction of the Homeless Program Section Chief and Chief of Social Work Service. The CERS liaison senior social worker is responsible for monitoring and coordinating services provided by community-based programs funded under VA's Homeless CERS Program. The CERS Program is vital for providing safe transitional housing and supportive services for homeless Veterans. VA medical centers must ensure the Veterans in community-based funded programs are receiving quality services. The CERS liaison senior social worker is responsible for the oversight of these services as well as the case management of the Veterans who receive them as identified in the contracted Performance Work Statement (PWS). The CERS liaison works closely with the Contracting Officer Representative (COR) for the CERS program. Additional Duties include but not limited to: Provides case management and oversight at the CERS funded site as described in the CERS program rules, regulations, and directives. Utilizes various treatment modalities of individual and group work, casework, community organization, and community outreach activities to alleviate or remove problems that interfere with the Veteran's ability to achieve independence. Performs specialized treatment of complex physical or mental illness. Incorporates complex multiple causations in differential diagnosis and treatment of Veterans including formulating psychosocial and psychiatric diagnoses within the approved clinical privileges or scope of practice. Coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery by linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies. Complies with criminal conflict of interest laws and executive branch standards of conduct as to avoid conflicts of interest while serving as the CERS liaison. Monitors clinical care and assesses the compliance of the programs receiving payment of services as described in the CERS contract. Collaborates with the COR to facilitate compliance or corrective actions and, when appropriate, develops, facilitates and monitors plans of correction. Provides services and oversight of the CERS funded program as defined in the CERS rules, regulations, and directives including verification of Veteran status and eligibility for participation in the CERS Program, maintaining admission, discharge dates of participants for billing purposes, reviewing and approving/disapproving readmissions, extensions of care, and collecting/submitting participant information as outlined by HOMES evaluations procedures. Completes weekend/holiday coverage twice annually for the inpatient medical units within Social Work Service. Work Schedule: Monday through Friday, 8:00 am to 4:30 pm, with rotating tours of duty as needed.
