Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Responsibilities This position is located at South Texas Veteran's Health Care System in San Antonio, TX (STVHCS). Primary Care Behavioral Health Clinic is an outpatient service in which patients are seen by appointment and same day with consults typically received from Primary Care. This program's mission is to improve access to mental health services and to try to provide such service on a same day basis while veteran is being seen in the primary care clinic. Duties include but are not limited to the following: Completes a functional assessment with each Veteran referred clarifying the Veteran-identified problem, @contextual factors, and impact on functioning. Identifies a preliminary diagnoses and initial treatment plan goals. Utilizes evidence-based screening tools for Depression, Anxiety, PTSD, and Suicide Risk and utilizes those tools throughout therapy to assess progress. Engages in brief interventional therapies to Veterans experiencing mild to moderate mental health acuity (e.g. cognitive-behavioral therapy, motivational interviewing, and solution-focused therapy). Provides same day access, including crisis intervention services, seeking to address the cause as well as the presenting complaint. Demonstrates working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures including acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology. Provides consultation services for primary care providers (PCP) to maximize the effectiveness of their interventions in reducing health-risk behaviors and enhancing health-promoting behaviors, with a significant emphasis on a prevention-oriented model of care. Supports primary care provider decision-making, building on PCP interventions and improving PCP patient working relationships. Provides consultation services to other staff regarding the psychosocial needs of Veterans and the impact of psychosocial problems on health care and adherence to treatment plan. Evaluates own practice through professional peer review, case conferences and other organizational means. Communicates effectively, both orally and in writing, with people from varied backgrounds. Serves as subject matter consultation to colleagues and students on the psychosocial treatment of patients treated in the PCMHI program. Renders professional opinions based on experience and expertise, and will role model effective social work practice skills. May teach and mentor staff and students in mental health services and provide supervision for licensure or for specialty certifications. Participates in administrative tasks within the PCMHI program, such as, dispositioning consults and leading PCMHI team meetings. Independently identifies high-risk patients and provides clinical or case management services as warranted. Follows STVHCS standard operating procedures for evaluating and mitigating suicide risk to include: Completes the Columbia Suicide Severity Risk Scale (C-SSRS) annually or more often as indicated, the Comprehensive Suicide Risk Evaluation, and a Safety Plan via shared template in the Computerized Patient Record System (CPRS) on all patients at intake, with Veterans with suicidal ideation and on any Veteran whose record is flagged "High Risk for Suicide." Additionally, completes the Suicide Behavior and Overdose Report (SBOR) as clinically indicated. Grade Determinations: Senior Social Worker, GS-12 : Experience/Education. The candidate must have at least two years of experience 9 post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Preferred Experience: EBP Certification Prior experience in similar role or with population served. Clinical supervision certification. Quality or utilization management experience.
