Summary The Home Telehealth (HT) Care Registered Nurse (RN) functions directly to support the activities of the St. Cloud VA Health Care System Primary and Specialty Medicine (PSM). The RN is responsible for the planning, integrating, implementing, modifying, and evaluating the effectiveness of the Veteran's care. Responsibilities The Home Telehealth (HT) Care Registered Nurse (RN) functional role includes following the standardized care coordination model, incorporating case management principles if applicable, knowledge of processes related to referral and authorization and secure medical documentation exchange. The RN will coordinate chronic disease management and case management with Veterans from the comfort of their homes. The RN plays a vital role in providing care through phone calls and video visits while working onsite, focusing primarily on hands-off patient care. The ability to identify Veterans requiring disease management and the process to obtain these resources internal or external to the VA if applicable. Major duties include but not limited to: Encourage the appropriate use of health care services and strives to improve quality of care and maintain cost effectiveness on a case-by case basis. Assist the patient in the safe transitioning of care to the next most appropriate level. Strive to promote patient self-advocacy and self-determination. Advocate for both the patient and facility to facilitate positive outcomes for the patient, the health care team, and the facility. Proficient in understanding Reusable Medical Equipment (RME) guidelines and demonstrates competency in application of pertinent equipment RME processes. Supports implementation of PACT principles, participates in cardiology huddles and implementation of redesign projects focused on improving the patient centered experience, improving patient' overall health, well-being, and satisfaction with care while meeting/exceeding goals of the VA organization. Support agency mission and vision to include but not limited to HPDP, tele-health, and patient centered care. Demonstrates skills/knowledge/ability to provide both face to face and telephone care assessment/triage services. In Community Based Outpatient Clinics (CBOC), demonstrates proficiency in venipuncture and maintains competency in lab specimen processing if needed. Demonstrates proficiency in medication management, including accurate preparation, administration, patient education, and documentation. Provides initial and ongoing assessment of patients to identify needs and appropriate resources and/or interventions necessary for care management. Applies nursing process, seeks resources, and identifies critical factors that may impact desired outcomes on discharge, post hospitalization recovery, or health maintenance/improvement. Identifies patient resources and supports during anticipated declines in health. Seeks advice and direction from appropriately skilled and experienced RNs requiring complex assessment and intervention. Sets clinical care goals, short and long term, in collaboration with patient, provider(s), including the Primary and Specialty Medicine team, the Mental Health Care team, Community Living Center, and family members. Qualifications Basic Requirements: English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD). Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.)
