Responsible for managing clinical operations and services to meet the needs of patients and families. Reports to the Administrator. Supervises all staff working in the clinical operations department. This position, if delegated, has the authority to act in the absence of the Administrator when the Administrator is not available by any means of communication. This position is for our South Carolina office located in Bennettsville.
Qualifications:
Education: Minimum Requirements: Graduate of an approved school of nursing and duly licensed as a nurse; Preferred at Hire: Bachelors and Masters in Nursing Certification/Licensure (including annual requirements): Minimum Requirements: Valid current SC or Multi state nursing license, 2+ years of experience in hospice or home health, advanced computer skills, management skills, valid NC or SC driver's license
Job Duties and Standards:
1.Develops and implements the department goals, objectives, policies and procedures that guide and support the provision of services for department operations. Motivates staff to accept and practice the policies and procedures, goals, and objectives.
Responsible for all aspects of the management of the nursing and ancillary medical services for Hospice Homecare patients, including: a. interviewing, selecting, orienting, evaluating, and conducting disciplinary action for clinical staff. b. planning for the professional development of staff. c. ensuring staff (and self) complete annual employee health requirements and annual mandatory education. d. recommends a sufficient number of qualified and competent people to provide care/services. e. monitors staffing needs and utilization of assigned areas and directs any appropriate alteration. f. communicates schedule and/or any alterations to staff. g. in charge of homecare clinical programs and services. h. involves staff in decision making
Maintains positive physician relationships as observed from physician comments, informal observation of problem-solving with physicians, and feedback from the Hospice Medical Director/Physician Designee: a. communicates activities, programs, issues to physician/physician offices as applicable. b. acts as liaison between medical community and support staff. c. consults with and reports regularly to the Medical Director/Physician Designee regarding patient status.

Maintains employee relations within the department, as evidenced by the following: a. conducting performance appraisals and competencies according to established guidelines; b. submitting all required HR paperwork (i.e., disciplinary forms, change of status, etc.) to the HR director; c. conducting clinical staff meetings on a monthly basis; d. updating performance standards and job descriptions on an annual basis; e. dealing with disciplinary matters according to policy; f. maintaining positive employee relations within the department as observed from employee comments, informal operations of dealing with employee relations issues and feedback from the HR director.
Maintains all licensure and regulatory requirements for the department as evidenced by: a. demonstrating knowledge of, adhering to, and promoting Hospice and department safety regulations, disaster plans, emergency codes, infection control policies, fire safety regulations, and hazardous materials disposal; b. ensuring full compliance with Employee Health and Infection Control program, including annual surveillance program; c. accepting responsibility for the care and maintenance of agency equipment; d. maintaining current knowledge of legal issues that may impact department; e. supporting efforts to ensure corporate compliance, privacy and business ethics.
Coordinates clinical services, including nursing, and hospice aide services for Hospice when appropriate: a. responsible for the supervision of all clinical services staff; b. oversees and coordinates the care of patients/families by making patient assignments and scheduling nursing staff; c. oversees the development of a written, individualized plan of care for each patient/family; d. sees that patients who are referred are reviewed for compliance with admission criteria and clinical services take place in a timely manner; e. coordinates and leads clinical services staff and team meetings, or other informal conferences that relate to supportive care services; f. assists team members in establishing priorities and a plan of action and facilitates timely reviews; g. coordinates referrals to other agencies, as needed, as well as inpatient admissions/transfers; h. conducts frequent evaluations of the status of care, recommending changes to the IDT, as needed; i. encourages prompt and effective communication among all team members;
j. ensures that documentation of all clinical services rendered is placed in the medical record in a timely manner, that documentation on care plans is updated every two weeks, and that physician orders are signed and on the chart; k. participates in and monitors performance improvement activities; l. delegates tasks to other team members and staff, as appropriate; m. oversees the ordering of medical equipment, supplies and DME
Supervises all clinical staff and all nursing and medical services provided to hospice homecare patients, including: a. provides supervision for all nurses, hospice aides, and nursing assistants b. ensures 24-hour nursing coverage, including back-up coverage, when needed; c. makes periodic home visits to evaluate care rendered; d. provides direct patient care as a Registered Nurse, as needed, including on-call services; e. schedules and evaluates services provided by ancillary personnel and/or other agencies; f. presents and present at QAPI meetings.
Information Technology a. oversees the development of an annual IT hardware replacement plan; b. ensures hospice information system remains complaint with all required regulatory upgrades. c. Ensures completion of annual supervisory visits for all field RNs/LPNs to fulfill compliance with service line and regulatory requirements. Supports State Survey readiness and process d.
