Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Responsibilities Total Rewards of a Allied Health Professional This Practical Nurse position is in the Geriatrics & Extended Care services at the Edward P. Boland Department of Veterans Affairs Medical Center located at the Leeds, MA location. May be required to travel to other VA campuses and CBOC's. This position is a full-time at 40 per week. Duties may include, but are not limited to: Collaborates with the FTC/Nurse Manager, Telehealth Assistant Nurse Manager, Program Analyst, and Lead Home Telehealth Care Coordinator to maintain exceptional ongoing telehealth services and establishing new provision of telehealth care. Assists with development of policies and procedures related to telehealth to ensure compliance with directives and regulatory guidelines. Performs telehealth imaging, including tele-retinal screening, tele-dermatology, tele-wound care imaging and other telehealth programming as identified and referred by primary care providers or other providers. Provides wound care and dressing changes per provider direction during or following clinical video telehealth visits. Ensures patient education of wound care & dressing changes. Measures wounds using acceptable technique using one time use tool or advanced wound camera technologies. Utilizes CPRS Vista Imaging to document telehealth images in the medical record and all clinical observations/information/patient education. Serves as a telepresenter within scope of practice during clinical video telehealth (CVT) and VA Video Connect (VVC) encounters including tasks such as establishing and maintaining video conferencing connections, making introductions between patient site and provider site, and following instructions of a clinician to facilitate an effective clinical encounter. Prepares the telehealth patient room including equipment for examination, training, education, access, and establishing the online, video conferencing or other connections. Serves as a liaison and coordinates information between image readers, primary care providers, specialty care providers, patients, and other VA personnel. Installs, maintains, inventories and trains various staff on all video conferencing technologies, telehealth peripherals (i.e. stethoscopes, otoscopes), and scheduling software. Initiates any indicated follow-up services based upon findings of readers (including scheduling patients with VA clinics) and communicates need for follow-up to the patient. Provides normal findings to patient per facility policy and as agreed upon with teleprovider. Ensures that standardized competency-based trainings, in accordance with established Telehealth program policies and directives, are provided to all telehealth staff, telepresenters, and teleproviders. This includes functioning as a telehealth preceptor to conduct a review to ensure technologies and systems are functioning prior to training, and trainees have completed prerequisites; observing and validating skills for initial certification and on-going performance evaluation. Providing established remedial training as needed and reporting to Facility Telehealth Coordinator regarding ongoing performance. Maintain knowledge of current standard practices. Serves as an imager and data manager for store and forward telehealth (SFT) clinical encounters including tasks such as capturing images, collecting data and transmitting them electronically for clinical review in accordance with VHA national guidelines. Receives and Schedules requests for simple and complex intra-facility, and inter-facility telehealth appointments using an electronic resource-based scheduling system. Screens patients for telehealth modality and referring patients that do not meet established criteria to appropriate care. At this grade level, the LPN/LVN must have a broader knowledge of the patient treatment process and exercise judgment that demonstrates the ability to effectively intervene when appropriate. Supports the RN or MD/DO in monitoring the practice of unlicensed nursing or nonmedical staff in patient care delivery. In addition to responsibilities at the lower grade level, the LPN/LVN is responsible for monitoring the practice of non-licensed nursing and medical staff. Work of considerable difficulty and responsibility is performed under general supervision. Individuals in these positions are, to a considerable extent, expected to exercise independent technical judgments and to possess and apply a comprehensive working knowledge of the principles, art and science of practical nursing. The staff mix is such that employees at this grade level are expected to perform practical nursing duties of considerable difficulty and responsibility under very general supervision.
