Summary The Military to Veterans Affairs (M2VA) Program Manager (PM), Registered Nurse (RN), supervises all transition and care coordination activities for post-9/11 service members and veterans receiving treatment at the facility. This role involves overseeing patient education, self-management support, and ensuring high levels of customer satisfaction across the continuum of care. The PM collaborates with interdisciplinary team members to positively influence care outcomes. Responsibilities The Military to Veterans Affairs (M2VA) Program Manager (PM), registered nurse (RN), ensures that severely injured and/or ill post-9/11 service members and Veterans with identified case management needs are assigned to appropriate case managers. The PM collaborates with an interdisciplinary team, including but not limited to, the Visual Impairment Services Team (VIST) Coordinator, Spinal Cord Injury (SCI) Coordinator, Polytrauma Case Manager, and Women Veterans Program Manager (WVPM). The M2VA PM serves as the primary point of contact within the facility for inquiries related to the transition of post-9/11 service members and Veterans, as well as those seeking healthcare services from the facility. The role includes addressing Congressional inquiries and managing action items from the Veterans Integrated Service Network (VISN) and the Veterans Affairs Central Office (VACO), as requested. The PM coordinates comprehensive program components and services, including preventive mental and physical health evaluations for program participants. They facilitate and promote evaluations for the Post M2VA Program at the facility and at Community-based Outpatient Clinics (CBOCs). The PM prepares and delivers program reports and is responsible for managing referrals between the VA, military treatment facilities, and community hospitals to ensure appropriate and timely inter-facility transfers. Additionally, they oversee the registration of eligible Veterans for clinical, administrative, and outcome tracking purposes. The M2VA PM establishes performance metrics and outcome standards to ensure the quality and efficiency of the program, aligning with the overarching goals of the facility, VISN, and VACO. They evaluate and document the program's effectiveness and continuously seek opportunities for improvement. The PM conducts research on community resources to facilitate continuity of care and improve Veterans' quality of life. They identify unmet needs within the post-9/11 population and work collaboratively with staff at the facility, VISN, and VACO levels to develop and implement effective solutions. The role also involves developing a data-driven continuous quality improvement (CQI) program with specific goals and measurable outcomes. Furthermore, the PM designs and executes outreach strategies to reach post-9/11 service members and Veterans who could benefit from VA services but are not currently utilizing them. They oversee the daily operations of the M2VA Program at the facility level, ensuring program effectiveness, and implementing service modifications to enhance customer satisfaction. Preferred Experience: A minimum of two years of experience as a registered nurse, including at least one year working within the Department of Veterans Affairs in a multidisciplinary team environment. Proven ability to collaborate effectively across multiple disciplines. Experience delivering presentations to groups, both in person and virtually, on the Military to Veterans Affairs Program. Qualifications Basic Requirements: English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD). Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.)
