Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Responsibilities Duties to include but not limited to: Clinical Functions Coordinates all clinical pharmacy functions for his/her assigned practice area, helping to achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams. Attends and participates in multidisciplinary rounds, team meetings, conferences, and other scheduled events as required. Designs, implements, assesses, monitors and documents therapeutic plans utilizing the most effective, least toxic, and most economical medication treatment for patients consulted to his/her care and other clinics as designated by supervisor. Performs initial and periodic assessment of patients' medication therapy, monitors as needed, and makes recommendations or changes (within scope of privileges) as necessary. Frequency and timeliness of reviews will be determined by The Joint Commission, and/or other regulatory requirements. Educational Responsibilities Demonstrates knowledge of the standards related to Hospital services and Acute/Critical Care unit drug use. This includes, but is not limited to, the standards established by DEA, FDA, VA, the state, and The Joint Commission. Serves as an authority on drug usage, interactions, over dosages, and compliance to medical staff, nursing staff, technicians, aides, students, and pharmacists of lower grade. Keeps abreast of changes in the Pharmacy Service (i.e. formulary additions and deletions, interdepartmental memoranda, relevant station memoranda, and policies and procedures) and maintains a general knowledge of VA, DEA and The Joint Commission regulations. Serves as a clinical pharmacy preceptor for Doctor of Pharmacy students from the appropriate affiliated Schools/Colleges of Pharmacy. Serves as a preceptor for PharmD residents and maintains a sustained record of contribution and commitment to pharmacy , as defined by ASHP. Administrative Functions Participates in medication use evaluations activities, audits or quality management monitoring as designated by supervisor. Gathers data for administrative reports when assigned. Upon request, independently prepares reports or participates in pharmacy projects. Assists in preparing for special accreditations and reviews (e.g. 5 Joint Commission, Inspector General, etc.,) and meets with reviewing/inspecting officials and pharmacy management. Performs at least one hospital committee assignment per year, or attends professional meeting and presents a report to the facility's professional staff and documents committee contribution or impact of meeting. Research Participates in medication use evaluations activities, audits or quality management monitoring as designated by supervisor. Gathers data for administrative reports when assigned. Upon request, independently prepares reports or participates in pharmacy projects.
