Summary The Home-Based Primary Care (HBPC) Supervisory Social Worker serves as a core member of the interdisciplinary HBPC treatment team which is a specialized program under the administration of Geriatrics and Extended Care. The HBPC Supervisory Social Worker works at the advanced level to provide expert psychosocial interventions and case management services to veterans with complex medical, social and behavioral conditions in which routine clinic-based care is not effective. The goal of the in-home assessment is to highlight the veteran's strengths, limitations, and internal/external supports and service needs in order to optimize the veteran's functional status and safely maintain the veteran in his/her home. Makes psychosocial diagnosis: Incumbent possess the advanced practice skills to independently make a psychosocial diagnosis and offer psychosocial interventions, and measures outcomes to interventions. The Supervisory Social Worker reviews data, subjective and objective, and makes a clinical assessment to identify needs and strengths. Makes independent professional decisions and recommendations for agency action. Incumbent is able to coordinate community-based services, including information and referral for additional services from other VA programs, other government programs and community outreach programs. Plans effective treatment: Independently identifies and prioritizes high-risk patients and provides case management services. Incumbent provides crisis intervention services, seeking to address the case as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Has working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology. Implements treatment: Will provide the veterans and their caregivers with ongoing supportive counseling utilizing the advanced knowledge of and mastery of theories and modalities. Gives advice, guidance, emotional support and other assistance to veterans and their families. Evaluation, education, supervisor, coaching and mentoring: Will collaborate in the performance improvement processes and support performance measures as required by the VA for their specialized population and/or program. Supervisory Social Worker, GS-12 Education and Experience: One year of experience equivalent to the GS-11 grade level.
