Summary The Social Worker - PACT/Specialty is assigned to the Jacksonville University Blvd Clinic within the North Florida/South Georgia Veterans Health System (NF/SGVHS). NF/SGVHS is a highly complex level 1a facility. NF/SGVHS consists of 2 medical centers, 3 large outpatient clinics, and 9 community-based outpatient clinics across 50 counties, of which 19 are in Southern Georgia. NF/SGVHS serves approximately 135,000 unique Veterans each year. Responsibilities The North Florida/ South Georgia VA Healthcare System Social work Service is seeking an experienced and dynamic Social Worker- Patient Aligned Care Team (PACT)/Specialty at the Jacksonville Clinic. The Social Worker - Patient Aligned Care Team (PACT) performs comprehensive psychosocial assessments, discharge planning, counseling with individuals/families/groups, provides education to Veterans/families, coordinates and arranges community based services, facilitates VA non-institutional care services, participates in multidisciplinary teams, provides advocacy for Veterans, provides consultation and education to staff regarding psychosocial aspects, and represents the mission of the VA in all interactions. Duties and responsibilities include, but are not limited to: Conducting psychosocial/clinical assessments of Veterans in collaboration with the Veteran, the interdisciplinary treatment team, family members, caregivers, and/or significant others in order to identify needs and formulate a plan to meet those needs. Evaluating the need for Veterans to be referred for capacity exams (neuropsychological assessments, payee, fiduciary, etc.) through the Primary Care or Mental Health Provider. Identifying family/caregiver stressors and making referrals to both VA and community resources to reduce caregiver burnout and support families and caregivers in providing care for Veterans. Assessing for any signs or symptoms of abuse, neglect, or exploitation and following required local policies regarding mandatory reporting and documentation. Providing case management and care coordination to facilitate appropriate delivery of health care services, incorporating complex multiple causation in differential diagnosis and treatment of Veterans and making psychosocial and/or mental health diagnoses within approved clinical privileges or scope of practice. Reviewing and coordinating all phases across the continuum of care. This includes primary care, specialty care, mental health care, acute care, extended care, and others. Educating the Veteran, their family members/significant others/caregiver and the interdisciplinary treatment team of all the options available to them to live in the least restrictive environment of care. Proactively addressing advance directive education and facilitating completion of advance directives, when appropriate. Assisting in the development of the psychosocial treatment plans, developing achievable treatment goals with the Veteran and family members/caregiver. Providing clinical interventions across a continuum of care to include supportive counseling, behavioral health counseling, and targeted psychotherapeutic interventions. Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. GS-9 Social Worker Experience, Education, and Licensure. None beyond the basic requirements. GS-11 Social Worker Experience and Licensure. Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level. OR, Education. In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting. Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, educational, and other backgrounds. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Preferred Experience: P Preferred years of experience in a medical and/or mental health setting with experience being in case management and care coordination for at least 3 years.
