Summary The Pulmonary Lung Cancer Screening RN Care Coordinator serves as a key member of the Pulmonary and Oncology care teams, providing comprehensive coordination and management of care for Veterans eligible for lung cancer screening services. The RN Care Coordinator is responsible for ensuring that Veterans receive timely, evidence-based, and patient-centered care across the continuum of lung cancer screening, diagnosis, and follow-up at the San Francisco VA Medical Center. Responsibilities The RN functions as a clinical expert, educator, and advocate, supporting Veterans through the screening process, ensuring adherence to national VA Lung Cancer Screening Program guidelines, promotion early detection and prevention strategies. The RN collaborates with multidisciplinary teams including radiology, pulmonary medicine, oncology, primary care, and surgery to coordinate diagnostic testing communicate results, and facilitate continuity of care. The RN Care Coordinator will: Assess Veterans for eligibility for lung cancer screening according to VA and national guidelines. Review medical history, smoking history, and risk factors to determine appropriate screening recommendations. Obtain informed consent and provide education regarding screening procedures, risks, and benefits. Serve as the primary point of contact for Veterans enrolled in the lung cancer screening program. Coordinate scheduling of low-dose CT scans, follow-up imaging, and specialty referrals. Review screening results and communicate findings to patients and providers in a timely manner. Track abnormal findings and ensure adherence to follow-up imaging, biopsy, or treatment recommendations. Maintain accurate records in electronic health systems and registries. Educate Veterans about lung cancer risk factors, prevention strategies, and lifestyle modifications. Provide counseling and support to Veterans with abnormal findings or diagnosis. Serve as a resource to answer patient questions and address concerns throughout the screening process. Participate in continuous quality improvement initiatives to enhance lung cancer screening services. Collect, analyze, and report data on screening outcomes, adherence, and program effectiveness. Identify gaps in care and develop solutions to improve patient outcomes and workflow efficiency. Actively participate in multidisciplinary meetings, case conferences, and community practice discussions. Share best practices and provide input on policy, protocol, and clinical pathway development. Maintain current knowledge of evidence-based practices, VA policies, and national guidelines for lung cancer screening. Ensure documentation meets VA standards for clinical, regulatory, and reporting requirements. Support accreditation, audits, and inspections related to lung cancer screening programs. Qualifications Basic Requirements: English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD).
