Perkins Hospital Center in Jessup, MD. The responsibilities for this position include provided an advanced level of clinical pharmacy services at the professional level focusing on medication use in patients and populations. Clifton T. Perkins is a Maryland state-funded, 250-bed, maximum security forensic psychiatric hospital. Reporting to the Director of Pharmacy, the advanced practice pharmacist will provide clinical, pharmaceutical services to inpatients with psychiatric disorders. Analyzes drug usage trends in large databases. Interacts with prescribers and other health professionals regarding appropriate medication use, medication safety and pharmacotherapeutic monitoring. Documents consultations and prescriber interactions, create reports that summarize clinical activity and present them to supervisors, reviewers and oversight panels. Serves as a preceptor for pharmacy students and residents, as assigned. Perform formulary and medication use reviews as assigned. Attends rounds or treatment team meetings. Directly interacts with patients to obtain health information, monitor medication effects, and communicate care plans. Participate in quality improvement services for accreditation, certification or maintenance of permits and licenses as needed. Review patient medication profiles; dispense and distribute medication. Perform administrative duties, such as organizing and directing technician's work flow, verify their preparation, labeling, order entries, and inspections, and coordination of drug utilization reporting. May participate in collaborative drug therapy management. Performs other duties as assigned. MINIMUM QUALIFICATIONS: Education:Bachelor's degree in pharmacy required. Preferred Education:Doctoral degree in pharmacy (PharmD) preferred. Experience:Two (2) years of experience in pharmacy,OR One (1) year post-baccalaureate or post-doctoral/post-graduate pharmacy residency (PGY1) plus one (1) year of experience,OR Two (2) years post-baccalaureate or post-doctoral/post-graduate pharmacy residency (PGY2). Preferred Experience:Residency in psychiatric pharmacy preferred. Experience in a direct patient care role preferred. Licensure/Certification:Current Maryland licensure in good standing to practice as a pharmacist or a federal district licensure where the work will be performed upon hire. Certification in a relevant specialty by the Board of Pharmacy Specialties or Commission for Certification in Geriatric Pharmacy Board such as Certified Psychiatric Pharmacist (BCPP) or Board Certified Geriatric Pharmacist (BCGP) eligibility preferred.
