Summary The License Practical Nurse is responsible for providing nursing care to patients of an aging, adult population. Incumbent demonstrates good clinical practice skills, problem-solving skills and decision-making skills and also provides a wide range of clinical and technical services to the Veteran under supervision of the Registered Nurse / Manager with Patient Care Services (PCS). Duties include but not limited to: Demonstrate comprehensive understanding of disease conditions, nursing principles and standards/practices to coordinate nursing care required in the clinic setting. Provides direct nursing care to adult and geriatric patients with complex problems according to nursing standards and Medical Center policy and procedure. This includes, but is not limited to: Checks patients into clinic, interviews patients and/or significant other and documents intake data per policy. Provides high quality nursing care using traditional and non-traditional modalities, including triage prior to provider face to face visits, nurse health maintenance visits, telephone triage, secure messaging, post hospital discharges, etc. Performs clinical pertinence reviews to ensure compliance with nursing practice standards related to nursing clinical reminders thoroughly and accurately per due dates and clinical appropriateness. Demonstrates knowledge, skill, and ability to promptly recognize and effectively intervene in potentially urgent or emergent situations including hypo/hyperglycemia, acute shortness of breath, acute chest pain, seizures, stroke symptoms, mental health issues, etc. Prepares, administers and documents medications and immunization as directed without error that may cause harm to patients. Demonstrates advance knowledge and skill in preparation, administration, and documentation of commonly prescribed oral, topical, subcutaneous, and intramuscular medications. Observes for subtle physical, emotional side effects, or contraindications related to medications when interviewing patients. Collaborates with RN, provider, and/or pharmacist to address patient medication questions and/or changes in patient conditions from prescribed medications/treatment. Assesses records and reports vitals signs, neurological signs, weight, and blood glucose levels. Appropriately triages and prioritizes patient needs as routine, urgent or emergent in the clinic setting and modifies patient care/treatment as needed. Serves as a resource for complex/specialized procedures (i.e. non-complex suture removal, non-complex wound care, blood draws and clinic specific procedures, such as ear irrigation) Performs venipuncture for blood draws, prepares specimens for transport and processing, transcribes orders, accessions labels, etc. Provides education to patients/families about preparation for procedures and /or diagnostic tests, labs. Teach patients and families the principles of health, self-care and treatments related to wellness and health promotion. Provides for a safe, clean, and therapeutic environment. Follows all safety policies and procedures including using good body mechanics and wearing personal protective equipment as needed. Communicates calmly, courteously, and professionally with all patients, personnel, and visitors while maintaining confidentiality in accordance with HIPAA regulations and Medical Center policies. Serves as a preceptor in orienting, educating, and training less experienced LPNs/LVNs or NAs/HTs related to support duties for these more complex, specialized tests/procedures. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, pushing, lifting, and transferring during administration of patient care. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures, including the ability to wear a N-95 respirator. Must be able to utilize good body mechanics and have stamina to withstand stress. English Language Proficiency. Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in [both] spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).The incumbent must be able to perform all activities for the position without restriction. This will include work on expanded tours of duty, weekends, and the ability to push supply, medication, and telehealth carts.
