This is accomplished in collaboration with the other members of the HBPC Team. Major duties include but are not limited to: Responsible for assessment, planning, and provision of case management services in the HBPC program. Coordinates a variety of activities including outreach, interdisciplinary activities, psychosocial assessments, treatment planning, consultation/education to Medical Center staff and community-based service providers, the collections and maintenance of statistical data, program planning evaluation and modification. Reassess veterans' needs for non-institutional, institutional services, programs, and entitlement. Educates veterans and their families of available services and assists with establishing the appropriate referrals. Provides veterans and their caregivers with ongoing supportive counseling to cope with chronic/disabling illness(es), onset of catastrophic illness and need for non-instructional services, as needed, during the course of the enrollment in the HBPC Program. Updates Veteran electronic records in a complete, confidential, and professional manner. Establishes positive working relationships and collaborates in the performance improvement process. Complies with performance measures as required by Veteran Affairs for their specifics population and/or program. AND Experience or Education Requirement: In addition to meeting basic requirements, to qualify for the GS-11, you must possess a minimum of one year of post-MSW experience equivalent to the GS-09 grade level in the field of health care or other social work-related settings, (VA or non- VA experience) and licensure or certification in a state at the independent practice level. Examples of specialized experience include but are not limited to: Working under close supervision in program areas that do not require specialized knowledge or experience. Identifying behaviors or symptoms of abuse, neglect or exploitation; providing education on advance directives and advanced care planning; providing social work case management; acting as an patient/family member/caregiver advocate with community service providers/agencies; assessing the psychosocial functioning and needs of patients and their family members.
