GI Advanced Practice Providers manage a full patient caseload and are trusted experts in this specialized field. They provide new patient assessment for patients with liver and general GI disease; ongoing chronic disease management for this patient population; and between-visit partnership with patients in the management of their disease. GI Advanced Practice Providers manage a full patient caseload and are trusted experts in this specialized field. They provide new patient assessment for patients with liver and general GI disease; ongoing chronic disease management for this patient population; and between-visit partnership with patients in the management of their disease. Performs other duties as assigned. MAJOR JOB DUTIES AND TASKS: Percentage Of Time: 1. Obtains history and examines patient 20% Reviews patient's records. Discusses patient problem with patient. Performs examination. Documents in patient record. 2. Orders appropriate labs and follow-up care 10% Documents orders in patient record. Notifies appropriate personnel to carry out orders. 3. Discusses with and instructs patient and family members regarding treatment and follow-up. 20% Documents in patient record. Provides written instructions to patients (patient education). 4. Communicates and follows up with patients in between clinic visits. 10% Reviews lab and test results. Makes recommendations for treatment next steps and modifications. May include Scheduled Telephone Visits. 5. Interacts with other healthcare professionals. 10% Obtains pertinent patient information. Obtains consults. Provides feedback or instructions. 6. Completes all required paperwork in a timely fashion. 10% Maintains and review patient records, charts, and other pertinent information. Posts tests and examination results. 7. Collaborates and coordinates with other health professionals. 5% Makes and accepts referrals to/from, consults with, cooperates with and refers to all other @types of healthcare providers, including primary and specialty care physicians, nurse practitioners and other allied personnel. Discriminates between situations requiring independent management, collaborative management with physician or further referral. Collaborates with physicians when unusual incidents or changes in patient condition occur. Coordinates care involving other healthcare providers. Functions as mentor/resource and educator for other healthcare team and staff. 8. Coordinates flow of Inpatient Service. 5% Reviews daily patient lists. Prioritizes consultations and rounding schedule according to established parameters. Develops and reviews work plan for the day with rounding physician partner. Communicates regularly with partner Inpatient APP to coordinate consult services and to balance workloads. Assists in the assignment of residents and students as necessary. 9.
