Summary The Clinical Pharmacy Specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. The primary practice area for this position is split with Academic Detailing (AD) and Community Living Center CPS duties. Responsibilities Major Duties include but are not limited to: Academic Detailing activity will encompass at least 20-50% (as required for the position) of the CPS's time. Cooperates and collaborates with the VISN 4 AD Lead to align campaign activities with VISN and local objectives. Develops policies and tools such as clinical guidance, dashboards, reports, clinical reminders, and consults to optimize disease management. Collects data and tracks use, implementation and impact of developed policies and tools. Analyzes prescribing practices and targets education campaigns to providers who are identified and may require additional focus to ensure prescribing adherence with current clinical practice guidelines. Meets with providers regularly, in brief one-on-one and/or small group meetings to discuss guidelines, review their prescribing profiles, and educate on relevant, newly published clinical literature. Provides lectures and/or in-service presentations related to clinical practice guidelines when requested by healthcare teams. Demonstrates an ability to work under a variety of circumstances to maximize group productivity and quality. Encounters (e.g. face to face and virtual) with referred patients, for comprehensive medication management in practice areas identified in their individual scope of practice. Orders, performs, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. Grade Determinations: GS-13: Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Pharmacists assigned to this position must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: minimum three years experience in VHA, PGY1 and/or PGY2 residency completion, BPS Board Certification, teaching certification/teaching experience.
