Summary This position collaborates with local Public Housing Authority (PHA), community partners, and VA Medical Centers (VAMC), to oversee a new or existing HUD-VASH project-based development serving aging or disabled Veterans who can benefit from an enhanced model of care. This position will be a subject matter expert in aging and disabled Veteran care, and maintain and create new referral pathways to assist aging and disabled Veterans with independent living. This position will supervise and oversee program development, policy, and treatment coordination for the aging and disabled Veterans participating in the enhanced model of care, including administrative and clinical supervision of a multi-disciplinary team. Must hold a full, current, unrestricted licensure as a Social Worker in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Education. Posses a Master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). GRADE DETERMINATIONS: In addition to meeting the above basic requirements, applicants must possess the below grade requirements to be found qualified for this position. Experience . One year of experience equivalent to the GS-11 grade level. AND, Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Knowledge, Skills and Abilities (KSAs). In addition to the above grade requirements, applicants must clearly demonstrate the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice.
