Schedule: 1.0 FTE, with 36 Patient Contact Hours; 4 hours of admin time per week. Clinic hours are 7:00am to 5:00pm.
Call Schedule: On-Call provider takes nightly/weekend/holiday Call; rotation is 1:18.
Practice Detail: Ascension Medical Group-Richmond St Clinic in Appleton, Wisconsin is looking for a Family Practice Physician to join our elite team (4 MD/DOs and 3 Nurse Practitioners-fully staffed). This clinic is a thriving environment and providers see 12-18 patients per day. Our clinic is conveniently located, off hwy 41, in Appleton. We have Radiology, Lab, Physical Therapy and Urgent Care onsite. Will consider new grads.

Ascension-Wisconsin is looking for a Board Certified/Board Eligible Advanced Practice Provider for our Primary Care-Family Medicine Team.

Responsibilities:
Obtains patient history and performs assessment via observation, interview and examination. Orders, performs and interprets diagnostic studies. Performs preventative health assessments, screening, immunizations and patient care.
Provides direct treatment and management of health conditions via referral to other healthcare providers or community resources.
Counsels and educates patients and their families/caregivers concerning preventative health, treatment options and community resources.
Documents delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.

Requirements
Licensure / Certification / Registration:
BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Minnesota Board of Nursing is also accepted. Licensure is dependent on state associate works in. Verified by local credentialing office.
Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Minnesota Board of Nursing OR current home state license, if considered multi-state/Compact State, also accepted. Licensure is dependent on state associate works in. Verified by local credentialing office.
Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred. Verified by local Credentialing Office.
Education:
Master's degree of Nursing required.
