(Junction City- Campus) This position is a clinical role dedicated to
supporting Interdisciplinary Teams (IDTs) in developing, implementing,
and evaluating patient-centered treatment plans. As a core member of the
treatment team, the Treatment Care Plan Specialist is responsible for
coordinating all IDT meetings, leading the development of Treatment Care
Plans (TCPs), and ensuring the integration of all aspects of patient
care into the TCP. This includes incorporating Behavior Support Plans,
Emergency Treatment Care Plans, STARTs, and other key assessments to
create a comprehensive and individualized approach to care. If this

Hospital\' video here. What We Are Looking For Minimum Qualifications:
These qualifications must be visible in your application for
consideration. A bachelor\'s degree in business or public
administration, behavioral or social sciences, or a degree related to
the agency program that demonstrates the capacity for the knowledge and
skills; and two years of experience coordinating or administering a
program OR Any combination of experience or education equivalent to five
years of experience that typically supports the knowledge and skill
requirements listed for the classification. Desired Attributes:

integrating assessments, behavior support plans, and risk data into

Strong written and verbal communication skills to lead IDT meetings,
keep teams on track, and effectively engage patients, families, and
staff. Attention to Detail & Organization \' Ability to manage complex
scheduling, documentation, compliance with hospital policies, and ensure
accuracy in treatment care planning. Cultural Competence &
Patient-Centered Approach \' Demonstrates respect for diversity and
commitment to developing individualized, recovery-focused, and
culturally responsive care plans.
