Full Time Home Care Physical Therapist

As a home care Physical Therapist, you'll provide exceptional one-on-one care to patients in the comfort of their own homes. Focused on patient-centered treatment, you'll work closely with a collaborative team-including physicians, nurses, social workers, and home health aides-to deliver the highest standard of care.

What You Will Do:

Deliver Personalized, One-on-One Care : Build meaningful relationships with patients in their homes, guiding them through recovery and promoting independence.

Minimum Qualifications:

Education : Bachelor's degree with progressive experience or a Master's degree in Physical Therapy.

Licensure : Must be licensed to practice as a Physical Therapist in Illinois.

Experience : Previous experience in home healthcare or familiarity with conditions of participation is preferred.

Additional : Must have a current driver's license and reliable transportation to travel to and from patient sites.
