General Summary
Working as part of a leadership team, is responsible for delivery, efficiency and effectiveness of patient care services throughout the WellSpan VNA service area. Provides supervision and direction for Home Health/Hospice Clinical Supervisors and participates in activities relevant to the patient services provided. Develops and manages staff and fiscal resources to ensure that high-quality, cost-effective patient care services are delivered in an efficiently operated system. Assists in evaluation of current service delivery, identifying strategic service needs, and participating in the planning, development, and implementation of new programs. May assume the functions of the VNA Director-Clinical Operations in his/her absence, as delegated.
Duties and Responsibilities
Essential Functions:
Related to Staff Participates and support Home Health/Hospice Supervisor in the hiring, orientation, development, coaching, corrective action and termination of personnel as appropriate with supportive documentation. Ensures adequate Home Health/Hospice Supervisor education and evaluation by providing direction to educational and teaching efforts, fosters environment for self-development and learning of staff, and approves continuing education. Prepares annual performance evaluations of Home Health/Hospice Supervisors and any other direct reports. Ensures adequate, competent and qualified staff to deliver scope of care provided by the agency. Ensures ongoing process to assess, maintain and improve staff competency. Demonstrates leadership by example while promoting "Just Culture" and positive interactions throughout the organization.
Related to Home Care ManagementFollows Conditions of Participation to ensure all regulatory requirements are met.Ensure processes for 5 elements, best practices, and change of status are followed. Reviews region/zone scorecards in SHP to ensure 4.5-5 star quality care including rehospitalizations. Organizes and directs the office's ongoing function, assuming responsibility for services and assists in the development and implementation of policies and programs. Participates with In Episode Management and ensures compliance with regulatory standards. Communicates and problem solves with VNA Leadership on patient care and management issues. Maintains an awareness of trends and interprets federal and state regulations applicable to all VNA service lines in addition to the state practice acts. Implements policies and procedures. Supports Performance Improvement initiatives throughout the fiscal year to support Bold Goals. Collaborates with VNA Leadership in identifying potential referral sources. Establishes and maintains positive working relationships. with current and potential referral sources. Reviews all S1 and CRS for 4 C completion, countermeasures, and service recovery.
Related to Community Relations * Assists in planning, implementing and evaluating field experience of professional students in the home health program, as requested.
Related to Corporate Management Provides assistance in community education, as requested. Represents the agency in a professional manner with community and professional organizations, as well as System partners. Participates in committees and in the process of policy and procedure development. Functions in an efficient cost-effective manner with regard to utilization of time, supplies and equipment. Accountable for seeking consultation, as needed. Carries out special projects and duties, as assigned.

Qualifications
Minimum Education:
Diploma Program Required or
Associates Degree Required
Bachelors Degree Nursing (BSN) Preferred
Work Experience:
7 years Clinical experience. Required
3 years Home Care/Hospice experience. Preferred
Licenses:
Licensed Registered Nurse Upon Hire Required or
Registered Nurse Multi State License Upon Hire Required or
Licensed Physical Therapist Upon Hire Required or
Licensed Occupational Therapist Upon Hire Required or
Licensed Speech-Language Pathologist Upon Hire Required and
Basic Life Support Upon Hire Required
