What Success Looks Like In This Job
The Adams County Health Department Nursing Division is comprised of Nurse Family Partnership, Nurse Support Program, Harm Reduction, Sexual Health, Immunizations, Child Maltreatment and Fatality Prevention, and Care Coordinator for Children and Youth with Special Health Care Needs. The Nursing Division is made up of clinical support staff, medical assistants, harm reduction specialists, operations support, nurses, advanced practice nurses, and midwives.

The Public Health Nurse (PHN) Coordinator - Immunizations provides leadership for Adams County Health Department's Immunization Program through oversight of the Vaccines for Children (VFC) program, vaccine inventory management, storage and handling, and nursing practices. The position serves as the primary nurse trainer and program specialist, ensuring staff are equipped to deliver safe, evidence-based, and culturally responsive immunization services. Responsibilities include developing and reviewing program procedures, supporting program operations, and maintaining compliance with CDC, CDPHE, and agency guidelines. The Coordinator also delivers direct patient care in clinics, during home visits, and at community outreach events.
In addition to clinical duties, the PHN Coordinator supports program planning, evaluation, and quality improvement efforts while building and sustaining community partnerships. This includes working with schools, health systems, community organizations, and public agencies to improve access to immunizations, reduce barriers to care, and increase vaccine uptake, especially among marginalized populations. The role serves as a public health resource and community expert on immunization best practices, contributing to the prevention of vaccine-preventable diseases and the promotion of population health across Adams County.
This position requires evening, weekend, and occasional on-call duties.
Examples of Duties for Success
Direct Patient Care
Provide safe, evidence-based, and culturally responsive direct patient care, including immunization record screening, client education, and vaccine administration for all age groups in clinics and community settings.
Maintain current knowledge of vaccine recommendations, storage, handling, safety, and administration practices in compliance with CDC, CDPHE, and agency guidelines.
Document care accurately and timely in the Electronic Health Record (EHR) and ensure adherence to clinical protocols, cold chain management, and safety standards.
Collaborate with internal teams and external partners to coordinate care, provide referrals, and link clients to available health services.
Participate in epidemiological investigations and public health response activities related to infectious diseases and vaccine-preventable illnesses.
Program & Community Leadership
Oversees the management of the VFC program acting as the primary VFC coordinator.
Oversees vaccine inventory management including but not limited to ordering, receiving, return of vaccine and ensuring cold chain management.
The Immunization Public Health Nurse Coordinator attains knowledge and stays up to date, maintains clinical competence that reflects current nursing practice.
Support program planning, evaluation, and quality improvement efforts, including tracking outcomes, conducting needs assessments, and aligning work with public health accreditation standards.
Provide leadership within the immunization program by mentoring team members, supervising staff when assigned, and fostering a culture of learning and collaboration.
Plans, develops, manages, and implements public health nursing programs for communities, families, and individuals; and performs related work as required.
Training
Develops and oversees robust staff development, including orientation, competencies, and training and professional development opportunities to maximize program delivery.
Assists in the orientation and training of new employees, students, and volunteers.
Participates and ensures implementation of quality improvement projects.
Assures staff compliance with ACHD and program directives, procedures, and guidelines.
Qualifications for Success
Licensed Registered Nurse (RN) in the State of Colorado; BSN preferred.
Strong foundation in nursing practice, including patient assessment, treatment, immunization administration, and documentation.
Knowledge of public health principles, including population health, epidemiology, and social determinants of health.
Demonstrated ability to apply health equity principles and cultural humility when working with diverse communities.
Experience collaborating with community-based organizations, schools, government agencies, and health systems.
Effective verbal and written communication skills, with the ability to adapt messages to varied audiences and settings.
Skilled in patient and community education, including the ability to explain complex medical information in clear, accessible language.
Ability to exercise tact, discretion, sound judgment, and problem-solving in clinical and community settings.
Proficiency in using Electronic Health Records (EHR) and maintaining accurate, timely documentation.
Experience coordinating or participating in outreach, community events, or school-based health initiatives.
Ability to analyze data, track program outcomes, and contribute to quality improvement and evaluation efforts.

Experience:

Education and Training:
