Summary The LTC Charles S. Kettles VA Medical Center is seeking a Full-Time Licensed Practical Nurse float for the PACT Clinics in Ann Arbor and Flint . This LPN is aligned under Ambulatory Care Services, and will be assigned to various Patient Aligned Care Teams(PACT) and/or Sub-Specialty Clinics located at the Flint CBOC (75% or more) and Ann Arbor VA Medical Center (25% or less as needed). Responsibilities Total Rewards of a Allied Health Professional The LPN is aligned under Ambulatory Care Services and will be assigned to various Patient Aligned Care Teams (PACT) and/or Sub-Specialty Clinics. This is a float position with the official duty station designated as the Ann Arbor VAMC. To support overall health care delivery efforts of the VA Ann Arbor Healthcare System (VAAAHS), the LPN may be assigned to one or more clinical areas within the medical center, community-based outpatient clinic (CBOC), and/or offsite ancillary clinic to perform clinical roles, within the scope of their practice, when determined clinically necessary. Some of the areas/clinics the LPN would typically be assigned to are: Phlebotomy Lab, Women's Health, Integrative Pain, Immunization, Foot Clinic. Duties Include, but are not limited to: Performs assigned nursing care responsibilities, under limited to moderate supervision, including use of equipment, technology, and/or knowledge in procedures performed in the PACT/Sub-Specialty Clinics. Documents vital signs including pulse, blood pressure, respirations, temperature, and pain level. Completes clinical reminders for preventive/mental health screening. Performs diabetic foot screening including visual screening, pulses, and sensation, pulses: apical, carotid, and pedal, height/weight. Administers influenza, pneumococcal, tetanus, COVID-19, and other vaccinations per protocol. Refers patients to appropriate resources including social work, dietitian, speech pathology, SAC, smoking cessation, and physical and occupational therapy. Performs clinical measurements as routine or specific to clinic area needs/standards, including: Glucometer measurements, neurological vital signs, performing EKG. performing pulse oximetry, bladder scanning, sterile dressings, irrigations, and post void residual. Adheres to standards of practice and safe technique in preparation and administration of medication. Procures sterile and non-sterile specimen collections. Prepares equipment administers Oxygen Therapy, applies bandages, hose, 4 layer compression. Provides patient/family education for patient's condition, procedures, and preventive health care; reinforces health teaching and promotes healthy lifestyles. Communicates with difficult patients under limited to moderate supervision. Adheres to basic principles of asepsis, safety, and universal precautions to provide a safe environment and possesses knowledge of proper handling of instruments including cleaning and sterilization. Participates in training setup, use, reprocessing, and maintenance of their clinic/department specific materials/equipment. Provides supportive care to patients for Intake and admissions, handles referrals from other VAMCs or Non-VA clinics, transfers patients from stretcher to chair/stretcher/wheelchair, applies restraints and protective devices, transporting/ escorting/errand running within designated clinic to support continuity of patient care, provides instructions to patients on how to prepare for testing and/or procedures, provides patient education, and monitors/maintains/orders stock medication and supplies. Collaborates with interdisciplinary team to support the development, implementation, and evaluation of patient plan of care. Utilizes the computer to access patient data to verify appointments, review progress notes, and short medication profiles. Orders labs and/or looks up test results under specific clinic profile.
