Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Responsibilities The New Graduate Primary Care Advanced Practice Provider (APP) Program, in collaboration with the interdisciplinary team, manages the care of outpatients in the Ambulatory Care Service as a Primary Care Provider as part of a Patient Aligned Care Team. Rotations for this program include: Orthopedics, Cardiology, Heme/Onc, Diabetes, Geriatrics, and Primary Care. Preceptors and Mentors will provide new graduates with ongoing support throughout the program. The New Graduate Primary Care Advanced Practice Provider Nurse Practitioner will experience an extended on-boarding process over several months. At the conclusion of the on-boarding, the Nurse Practitioner, will function as a Float Primary Care provider. During the extended on-boarding process, the New Graduate Primary Care Nurse Practitioner will be expected to travel to Madison Central VA location for specialty rotations as needed. The Primary Care NP roles and responsibilities will include but are not limited to the following: - Face to face clinic appointments for follow up and acute patient care visits - Telephone appointments - Completing Patient history and physical evaluations - Review and follow up of diagnostic test results - Communication of test results to patients - Establishing ongoing care of plan for patients based on clinical guidelines and veteran Centered Care principles - Review and summary of non-VA records and incorporating into the ongoing plan of care Hand off communication to and from primary care and specialty, Emergency department and inpatient providers within the VA the community - Sending referrals to specialty consults as appropriate - Directing care coordination plans to PACT team members - Completes final medication reconciliation - Completes clinical reminders - Work as a member of the interdisciplinary Patient Aligned Care Team (PACT) and provide care to a panel of approximately 900 patients when covering a panel - Provide services as directed to meet the needs of the patient population being served by the medical center. Advanced Practice Nurse (Nurse Practitioner) Basic Requirements Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner. Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). NOTE: Foreign Education. Graduates of foreign schools of professional nursing programs, the nurse practitioner degree equivalency must be validated through an Agency recognized degree equivalency evaluation agency, such as the International Consultants of Delaware (ICD). Candidates with equivalent nurse practitioner masters or doctorate level degrees who possess current, full, active, and unrestricted registration and certification will meet the requirement of graduation from an approved school of professional nursing to warrant appointment. License and Certification: License. All APN (NPs) must possess and maintain an active, current, full, and unrestricted APN license to practice as an NP in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Basic Certification. All APN (NPs) must maintain full and current board certification in an area of clinical specialty from one of the following: the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners Certification Board (AANPCB), or other nationally recognized certifying body in the area in which the Nurse Practitioner is academically prepared.
