Summary The Program Coordinator for the Compensated Work Therapy-Transitional Residence (CWT-TR) is part of the Mental Health Residential Rehabilitation Treatment Program continuum at the Battle Creek VA Medical Center. This position serves as the Program Coordinator for the Compensated Work Therapy-Transitional Residence (CWT-TR) Program. This position reports directly to the Chief of Psychology Service but works collaboratively with the Vocational Rehabilitation Program. This program provides specialized professional mental health services that significantly impacts the health care provided to Veterans. Veterans in the program present with complex mental health, substance use, and psychosocial issues requiring a comprehensive, integrative approach to care. The components that are managed are of considerable scope, size, and complexity, and consist of a nine (9) bed Mental Health Residential Rehabilitation Treatment Program (MH RRTP) located in two (2) homes in the community. The specific responsibilities of the CWT-TR Program Manager include but are not limited to: provide oversight of facilities, safety, clinical operations, and participant selection. Improvement efforts are based on monitoring activities and Program reviews, and the development and analysis of information. In addition, the incumbent uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of Program enhancements. The incumbent performs Program analysis, development, review, research, design, and contact work as possible with various agencies, and VA Programs (Residential Programs, Vocational Rehabilitation, Homeless Continuum of Care, Homeless Coalition of Calhoun County, other public agencies, DOD, state, etc.). Have line and staff authority for resource allocations within the area of responsibility. Develop short- and long-range strategic goals and plans for the programs and implement policies and procedures to ensure goal attainment. Oversee and ensure program compliance with VHA MHRRTP, CARF, JCAHO and other regulatory and accrediting standards. Oversight for development and monitoring of the treatment program content to ensure that it is evidence-based and comprehensive enough to meet the biopsychosocial needs of all Veterans served by the program, regardless of age, gender, or warzone. Manage admission and discharge processes to ensure optimal patient flow and coordinated transition between levels of care. Facilitate cohesion and communication among the interdisciplinary team members from various services, programs, and clinics. Responsibly manage program financial and other resources to provide optimal service delivery to Veterans. Collect, aggregate, and analyze data to monitor treatment outcomes and to facilitate data-driven quality assurance processes; oversee and ensure completion of NEPEC data and other required program reports. Clinical Responsibilities (40% of time) include but are not limited to: Performs psychological consultations and evaluations in patients referred to CWT-TR specifically and MH RRTP generally. Provides a full range of therapeutic interventions including: individual therapy, couples/family therapy, and group therapy. Documents social work interventions and activities in patient's clinical record utilizing the electronic health record and ensure appropriate hand-off when transferring patient care. Is the primary point of contact for Veterans, caregivers, community agencies, and staff for information related to the CWT-TR. Establishes and maintains effective therapeutic relationships with Veterans and/or their families. Works with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, financial, legal and psychosocial problems. Other Significant Responsibilities (10% of time) Has contact on a regular and recurring basis with individuals representing widely divergent backgrounds, interests, and points of view. Serves as the Service Champion for various Mental Health initiatives as directed by the Service Chief. Participates in conferences/meetings with higher-ranking managers, supervisors, and staff from other disciplines or work units. May provide assistance with the orientation and learning of less experienced social workers and if interested, serve as a field instructor for graduate level social work students. Responsibilities include resource management and distribution, budget formulation and monitoring, assessing staffing levels and needs, space planning, and assessing equipment and technology needs. Serves on committees, work groups, and task forces at the facility as deemed appropriate by the supervisor and Chief of Psychology Service. Responsible for furthering one's own professional growth through continuing education and for ensuring continuing education appropriate to the area of assignment. Driver's License: Must posses a valid unrestricted Driver's License Grade Determinations Social Worker (Program Coordinator), GS-12 Experience/Education One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.
