Join a dedicated team in the Chippewa County Department of Public Health Planning and Strategy Division, committed to supporting children and youth with special health care needs (CYSHCN) and their families in Chippewa County. We are accepting applications for a Medical Services Screener position to help connect families to the services that play a vital role in building healthier futures. What You\'ll Do: As a Medical Services Screener, your responsibilities include but are not limited to: Complete Children\'s Long-Term Services Functional Screens in a variety of settings, including office, health department clinic, school, or home. Connect families to the Birth-to-Three program and other supports Collaborate with families, providers, and community partners to ensure responsive, professional service delivery As needed, provide intake, referral, and care coordination services to assist children and youth with special health care needs (CYSHCN) and their families, especially in the areas of mental health and other special health needs ** OTHER EXPERIENCE AND QUALIFICATIONS: Minimum Qualifications: Bachelor\'s degree in Social Work, Registered Nursing, or a related health care field is preferred, -OR- Associate\'s degree in a medical field or Licensed Practical Nursing with a minimum of two (2) years professional work experience required. Two (2) years or more of professional work experience in the mental health field is preferred Must meet statutory requirements to perform functional screens in the State of Wisconsin, including successful completion of Children\'s Long-Term Functional Screen training within two (2) weeks of date of hire. Must maintain professional credentials to practice in Wisconsin, if applicable. Experience working with families with children who have special needs is preferred. Valid driver\'s license and cell phone is required.
