Summary The Clinical Pharmacy Specialist (CPS) works in PACT/Primary Care at the New Braunfels VA Clinic. The primary responsibility of the CPS is comprehensive medication therapy management (MTM), evidence-based pharmaceutical care services through direct care with patients and consultation with other providers on their team. Will follow patients in direct care setting to achieve therapeutic goal. Clinical Pharmacy Specialists are employees of their medical center's Pharmacy Service, to ensure they receive all professional guidance, policy and communications, along with department missions and goals, especially regarding the VA National Formulary, VISN and local medical center Pharmacy and Therapeutic Committee objectives for formulary management activities and patient safety updates (i.e. notices, bulletins). The PACT/Primary Care CPS will perform their duties in Primary Care or may work remotely using telehealth technologies to include VA Video Connect (VVC) when the team requirements do not necessitate a full time PACT/Primary Care CPS in Primary Care be physically present. Monitors for and reports drug errors, adverse drug reactions, allergies, and patient compliance issues. Incumbent documents findings per facility procedures. Follow patients until they achieve the pharmaceutical therapeutic goals set by the provider, Chief of Staff or designee. Utilizes effective written and verbal communication techniques in teaching others. Actively involved in reviewing patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required as a provider. Communicates findings with other providers and provides appropriate alternatives to current treatment plans as needed. Assists in the development of proposals for improved and/or new clinical pharmacy services. Independently designs and conducts clinical research with the scope of practice. Assists in the training of new pharmacy staff. Serves as an authority on drug usage, interactions, over-dosages, and compliance to medical staff, nursing staff, technicians and students. Uses appropriate references to research drug information. Acts as a service and communication liaison between service line leadership, pharmacy management, medical staff and healthcare personnel and pharmacy team members. Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. 7402(f)). Grade Determinations: Experience. In addition to the Basic Requirements, you must have 1 year of experience equivalent to the next lower grade level (GS-12). Examples of GS-12 experience includes: handling routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; trucing health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution, OR Completion of an ACPE-accredited Pharm.D. program.
