Summary The Post 9/11 Military 2VA (M2VA) Program Manager oversees all transition and coordination of care activities for Post 9/11 Servicemembers and Veterans (SM/Vs) treated at the facility. Their health care and psychosocial problems and needs are complex and require a high degree of clinical oversight and creative problem solving. Ensures that severely-injured and/or ill post 9/11 SM/Vs and other SM/Vs with identified case management needs are assigned to a nurse or social worker case manager and oversees or works closely with care management teams to include nurse and social worker case managers, the Visual Impairment Services Team (VIST) Coordinator, the Spinal Cord Injury (SCI) Coordinator, the Polytrauma Case Manager if applicable, and the Women Veterans Program Manager. Serves as the facility point of contact for all inquiries regarding transition of post 9/11 SM/Vs and as point of contact for post 9/11 SM/Vs seeking health care from the facility. These duties include a wide variety of stakeholders (e.g., Veterans, families, state and community agencies, Veterans Service Organizations, military support programs, etc.). Incumbent addresses Congressional inquiries and Veterans Integrated Service Network (VISN) and Department of Veterans Affairs (VA) Central Office action items as requested. Coordinates comprehensive program elements and services for preventive mental and physical health evaluations for program participants. Promotes and coordinates evaluations for the M2VA Program anywhere within the facility and any outlying Community-based Outpatient Clinics (CBOCs). Prepares program reports for the VA Central Office, VISN POC, and Medical Center leadership. Monitors procedures for referrals between the VA medical center, Military Treatment Facility and/or community hospitals that promote appropriate and timely inter-facility transfers. Responsible for management of the Military Treatment Facility referrals and other identified post 9/11 SM/Vs located in the catchment area. Ensures registration of appropriate Veterans for clinical, administrative, and outcome purposes. Collaborates with Enrollment and Eligibility staff to ensure access to care. Establishes performance and outcome standards for the facility M2VA Program that promote quality and efficiency of services in coordination with the overall goals of the medical center and those of the VISN and VA Central Office, as well as evaluate and document the program's effectiveness. Monitors policy and procedures to ensure compliance with Joint Commission (JC), Commission on Accreditation of Rehabilitation Facilities (CARF), and other accrediting bodies and regulatory standards as needed. Researches community resources (local, state, and national) that are available to provide continuity of care and to enhance the quality of life of the Veteran. Disseminates information on resources throughout the VISN. Explores and analyzes community resources including family-friendly delivery systems. Identifies unmet needs of the post 9/11 population and works collaboratively with facility, VISN, and VA Central Office staff to develop and implement solutions. Develops a data-driven continuous quality improvement program with established goals and outcomes to evaluate and document the program's effectiveness. Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: Supervisory Social Worker, GS-12 Education and Experience. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as Supervisory Social Workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Ability to independently organize work, set priorities, and meet multiple deadlines. Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement. Ability to ensure provision of clinical social work services. Ability to provide staff training and development. Skill in interpersonal relationships in dealing with employees, team leaders, and managers Preferred Experience: 2 year post LCSW, 2 year Supervisory/managerial and coordinator experience.
