The Department of Developmental Services (DDS) is seeking a mission-driven, dynamic, highly organized individual to fill the role of Nurse Practitioner. The selected candidate will work directly at Southeastern Residential Services as the agency supports 200 individuals with a variety of health care needs. The Nurse Practitioner is part of the Core Clinical Team who is responsible for the management and oversight of 50 residential homes. The Nurse practitioner will ensure that individuals supported by Southeastern Residential Services receive comprehensive medical services and supports necessary to achieve and maintain an optimal level of good health. This is accomplished through a combination of clinical consultation, education and training activities, and coordination with generic health care system. Provide clinical supervision to the Agency Health Care Team by meeting regularly and providing consultation. Will provide support and consultation to various Health Care initiatives and projects as needed. Duties and Responsibilities (these duties are a general summary and not all inclusive): · Will function as the agency health care contact and will provide technical assistance to staff of the agency and to the residential homes. · Takes the lead in coordinating and overseeing critical medical decisions on behalf of individuals receiving DDS support. · Assists agency staff with hospital admissions and discharge planning. Will assist agency staff with interpreting medical terminology during crucial hospital/physical appointments on behalf of individuals receiving supports/services. · Consults with Agency and Area Office Staff regarding DNR's, Health Care Proxy issues and complex medical decision-making. · Work with Agency staff on implementation of the Health Care Initiative. · Provides training and information to agency staff on health care issues that pertain to the individual, and strategies for improving access to generic services. · Provides technical assistance in the areas of assessment, hospitalizations, treatment, and discharge planning for nursing facilities. · Whenever possible, will advise the Director of Residential Services or designee, with at least 24-hour notice, if there are plans for a nursing facility admission. · Consults to the ISP teams and Management Team with ideas for diversion from a nursing facility placement. · Works collaboratively with the Management Team for the individual to ensure all staffing, equipment needs are available upon return to the home. · Provides technical assistance in the areas of assessment, hospitalizations, treatment, and discharge planning for nursing facilities. · Whenever possible, will advise the Director of Residential Services or designee, with at least 24-hour notice, if there are plans for a nursing facility admission. · Consults to the ISP teams and Management Team with ideas for diversion from a nursing facility placement. · Works collaboratively with the Management Team for the individual to ensure all staffing, equipment needs are available upon return to the home. · Collects, monitor, review, provide input, evaluate, and report to the Agency Management Team information pertaining to medical issues and overall health care concerns and topics. · Review Medication Occurrence summary data with the Health Care Team. · Works with the Agency staff to review the coordination of individual consultations. · Reviews and provide follow-up to the Area Offices as requested on Mortality Reviews. · Provides medically related information through educational trainings to agency staff both at the Core office and residential homes. · Collects data to identify trends, areas of concern and to identify areas for long range planning, as directed by supervisor and Management Team. · Attends weekly meetings as needed to provide updates, review assignments, or gather feedback. · Meets with clinical staff (OT/PT/Speech/Respiratory Therapists) on a quarterly basis to review and follow up on individuals who have had clinical consultations. · Develops knowledge base related to support services available in the community system. · Collaborates with other medical professionals to develop this resource. · Networks with other state agencies to maintain knowledge of available services. Required Qualifications: · Knowledge of the principles and practices of nursing and psychiatric/mental health nursing including terminology. · Experience with standard nursing methods and techniques followed in administration of medication, injections, and preparations, and application of medical and/or surgical dressings, principles of physiological growth and development. · Strong knowledge of the Principles of physiological growth and development. · Ability to Interact with people under physical and/or emotional stress. · Ability to deal tactfully with persons from different ethnic, cultural, and/or economic backgrounds. · Strong Knowledge of regulations governing nursing in the expanded role. · Ability to make decisions and act quickly in emergency situations. · Exercise sound judgment, gather information by examining records and documents. · Capacity to observe individuals and draw conclusions, make recommendations; maintain accurate records; prepare general reports and apply laws, rules, regulations, policies, procedures, governing activities; prepare and use charts, graphs, and table. · Ability to make effective oral presentations; work cooperatively with families of individuals supported. Preferred Qualifications: · 4 years' experience as a registered nurse in psychiatric nursing or intellectual disability nursing work. · Experience providing nursing services to groups such as those with intellectual disabilities. · Knowledge of @types and application of emergency medical procedures such as cardio-pulmonary resuscitation and tourniquets. · Proficiency in MS Office including Word, Excel, PowerPoint and Access. · Strong Knowledge of the @types and symptoms of mental and/or emotional disorders. · Working Experience with @types and characteristics of drugs used in nursing care. · Ability to work independently in a fast-paced environment.
