Summary The Social Work Supervisor is responsible for the professional, clinical, and administrative oversight of Social Work staff across multiple locations, including the Main Campus, North Campus, East Campus, and Community-Based Outpatient Clinics (CBOCs). This role encompasses the comprehensive management of social work services within Primary Care Clinics, SCI/D and M2VA Programs, including program development, service coordination, and quality assurance. Responsibilities Supervisory Duties include but are not limited to: Responsible for all aspects of the provision of Primary Care, SCI/D and M2VA Social Work services throughout VASNHCS. Serve as the Subject Matter Expert (SME) for PACT, SCI/D and M2VA Social Work. Complete Ongoing Professional Practice Evaluations (OPPE) and Focused Professional Practice Evaluations (FPPE) for Social Workers within established time frames, to include assigning or completing corresponding chart reviews. Ensure that all PACT, SCI/D and M2VA social workers are assigned and evaluated on appropriate social work competencies within established time frames. Issue all performance plans and conduct mid-year and end of the year evaluations within established time frames. Initiate corrective action, in conjunction with Human Resources, when needed. Review and approve or disapproves leave requests. Collaborate with various services and departments that interface with Primary Care, SCI/D and M2VA Social Workers. Oversee provision of all Primary Care, SCI/D and M2VA Social Work services across the VASNHCS. Provide and arrange for on-going training for clinicians to ensure support and skills needed to accomplish assigned duties. Support and assist clinicians in maintaining appropriate clinical records, ensuring appropriate documentation, psychosocial assessments, treatment planning and discharge plans are completed in a timely manner according to station policy. Establish team goals, equitably distribute work assignments, and provide general direction for the work of team members. Provide programmatic guidance and feedback to supervised employees. Supervise and assist, where needed, in coordinating coverage and workload for PACT, SCI/D and M2VA Social Work teams/workgroups. Ensure adequate Social Work coverage across all Primary Care Sites, SCI/D and M2VA and initiate contingency plans when needed. Develop policies and procedures necessary to manage overall program operation. Prepare reports and statistics required for facility, VISN, and national use. Adjust work assignments and processes as needed to meet local, VISN, or national guidelines. Clinical Duties: In addition to supervisory responsibilities, the Social Worker will be assigned a PACT panel and will deliver direct patient care. The Social Worker will also provide cross-coverage for positions under their supervision, including M2VA, SCI, and PACT Social Work programs. Act as a care coordinator and case manager for psychosocial concerns related to psychosocial determinants of health for Veterans. Conduct psychosocial assessments in collaboration with the interdisciplinary treatment team, the Veteran patient, family members and significant others; interviewing Veterans and family members/significant others and writing the psychosocial assessment. Ensure biopsychosocial assessments highlight Veterans' strengths, limitations, and internal/external supports and service needs to optimize their functional status. Assess and treat the complicated psychosocial and behavioral health problems of Veterans, caregivers and their families/significant others. Assess, treat, and appropriately refer for the psychosocial problems that cause distress, which often impact the health condition and create barriers to care. Accept referrals from any member of the interdisciplinary team, Veterans, caregivers and/or family members who call or walk-in for assistance, and from community professionals. Complete comprehensive psychosocial assessments for patients to help determine what psychosocial and mental health services are needed and make appropriate referrals as outlined by medical center policy. Develop treatment plans in collaboration with the Veteran/family/caregiver, interdisciplinary treatment team, and appropriate community partners. Incumbent provides psychosocial interventions and measures outcomes to interventions. Provide therapeutic services to individuals, families, and groups covering a wide variety of psychosocial issues. Coordinate with PCMHI and/or specialty Mental Health clinicians after these referrals are made to ensure a continuity of care between the PACT and the Mental Health teams. Conduct psychosocial and behavioral health assessments in collaboration with the interdisciplinary treatment team, the Veteran, family members and significant others. Utilize a range of psychoeducation and psychotherapeutic treatment options based on Veteran needs and clinical appropriateness to address issues related to physical limitations, psychological needs and age-associated illnesses and conditions. Conduct applicable clinical reminders within their approved scope of practice or clinical privileges Provide crisis intervention, make rapid assessments and develop crisis management plans for Veterans when needed. Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: Supervisory Social Worker, GS-13 Experience/Education: One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as supervisory social workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs: Ability to independently organize work, set priorities, and meet multiple deadlines. Skill in a range of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement. Ability to ensure provision of clinical social work services by supervised social workers. Ability to delegate authority, manage priorities, coordinate work, and follow up on pending issues to complete duties in an accurate and timely fashion. Ability to analyze organizational and operational problems, develop and implement solutions that result in efficient operations, and use data effectively to manage workload, quality, performance, and productivity within the service.
