Occupational Therapist (OT)

Working alongside a multidisciplinary team, you'll help clients adapt to daily challenges and achieve their highest level of independence. Conduct in-home assessments of patients' physical, cognitive, and functional abilities Develop individualized treatment plans focused on improving daily living and motor skills Provide direct therapeutic interventions and adaptive equipment training Educate patients and caregivers on strategies to promote safety and independence Recommend home modifications and equipment as needed Collaborate with nurses, physical therapists, social workers, and aides for coordinated care Participate in care planning and team meetings as required Join our passionate team delivering compassionate, client-centered care across Rochester, NY and surrounding areas. Current New York State Occupational Therapist license Graduate of an accredited Occupational Therapy program Minimum of 1 year experience in home health, rehab, or clinical setting preferred Strong assessment, clinical, and communication skills Ability to work independently and manage time effectively Reliable transportation and willingness to travel to patient homes CPR certification and up-to-date health requirements
