About the Mental Health Transitional Living Homes
The CDHS Office of Civil and Forensic Mental Health's Transitional Living Homes Program was created to increase bed capacity for persons in need of residential behavioral health treatment. House Bill (HB) 22-1303 has authorized new beds to be used for persons needing residential services at a less restrictive level of care and to reduce and eliminate the backlog of such persons in more restrictive settings. HB 22-1303 directs the Department of human services (department) and the Department of health care and Policy Financing to create, develop, or contract to add at least 125 additional beds at mental health residential treatment facilities (treatment facilities) throughout the state for adults in need of ongoing supportive services. The bill also requires that the department renovate a building at the mental health hospital at Fort Logan to create at least 16 additional civil beds for such persons. These beds serve as a step-down option from inpatient care and as a step-up when deemed medically necessary and lower-level of care settings can no longer adequately and safely provide effective care.

About the Position:
This position facilitates and coordinates all admissions to and from the Group Homes programs. The Health Professional III screens referrals with minimal delay, pursuant to established Group Homes Placement Criteria. This position also creates a pre-assessment of referrals, which provides demographic, historical, legal, and clinical information. This position is responsible for tracking the total number of referrals compared to available beds, pending discharges from the Group Homes, and potential barriers for admissions and discharges.
The Director of Behavioral Health Group Homes delegates and supervises all duties assigned to the Health Professional III position. Duties may include, but are not limited to the following:
Taking referrals and jointly maintaining a current bed capacity list for the Group Homes from the Mental Health Hospitals.
Providing an assessment of collateral information, regarding patients referred for services to the Group Homes.
Collecting and disseminating patient information to the Group Homes clinical team in preparation for incoming admissions.
Constantly monitoring the Group Home bed availability census, including admissions, discharges, and continued stay requests in real time.
Duties in this position include:
Program Coordination/Management of Admissions & Discharges:
Collaborate with the various stakeholders, including mental health hospitals, treatment facilities, contractors, admissions program, legal records, and transport to ensure essential information is communicated as needed for admission, discharge, or continuity of care.
Demonstrates decision-making skills as it relates to the program's admissions schedule, involving short- and long-term planning.
Coordinates with the Group Homes Division Director and Group Home programs to support in maintaining all admission and discharge schedules, including transfers to and from various programs. This requires tracking of admissions, discharges, waitlists, and case status in order to anticipate the needs for each program, problem solve, and communicate to the various stakeholders.
Communication: keeps the Director of Behavioral Health Group Homes and team informed of the admissions, discharges, and transfers by coordinating those efforts, disseminating notification emails, disseminating relevant patient information/records, maintaining an admissions schedule, tracking data, and creating data reports.
Communication/Liaison:
Available to attend stakeholder and other departmental meetings.
Responds to inquiries and problem-solves as issues arise.
Facilitate communication among team members and outside agencies, while adhering to confidentiality/HIPAA standards.
Communicates pertinent patient updates regarding treatment status and/or location of treatment to the necessary individuals and collaborates in order to problem-solve as issues arise.
Demonstrate excellent communication skills by assisting others and representing the department in a positive and professional manner.
Identifies and meets the needs of internal and external customers, gives and receives feedback in an open and responsive manner, and collaborates with other departments.
Data Collection/Analysis and Reporting Projects:
Engage in daily tracking of various factors relating to the program (e.g., admissions, discharges, barriers to treatment and discharge planning, patient length of stay, reason for discharge, etc.), extrapolate that data, and generate data reports.
Assist with reporting to OCFMH and CDHS management about the program's performance and compliance with policies and procedures.
Analyze data, identify areas for growth, and solve problems to increase program efficiency.
Referrals and Admissions Coordination:
The Health Professional III oversees the admissions and discharges to and from the Behavioral Health Group Homes programs.
Respond to everyday requests regarding admissions and discharges.
Assist with the design and development of policy and procedures related to admissions, continued stay, and discharge to ensure the efficiency of department processes.
Works to ensure the rules of confidentiality are observed and followed.
Assists the Director in developing reporting mechanisms that show current referrals, admissions, and discharges in real time.
Ensures forms associated with admissions and discharges are updated and accessible, and serves as a role model and preceptor for other staff within the team.
Purchasing, Timekeeping, Travel, and Training:
Verifies that forms and tracking systems used by the department are in compliance with required standards, statutes, and regulations. As departmental needs change, they update the aforementioned forms and tracking systems to ensure accuracy and efficiency.
Schedule and attend annual mandatory in-service training, pursue continuing education skills, complete job-related competencies, maintain knowledge of the department, contractors, and competency-related statutes and relevant research.
Provides training to the department in areas of expertise as needed.

Six (6) years of relevant experience in an occupation related to the work assigned to this position
OR
Education and Experience:
A combination of related education ( Psychology, social work, criminal justice, counseling, or any other related behavioral health degree), and/or relevant experience in an occupation related to the work assigned equal to six (6) years.
Preferred Qualifications:
Previous state employee
Experience: Hospital, residential, group home, outpatient settings
Bachelor's degree in a related field and/or 1-2 years of experience in a residential or outpatient-based care setting - admissions, intake coordination, case management, or related behavioral health role.
Familiarity with mental health diagnoses and various care systems within Colorado. Excellent verbal and written communication skills. Ability to manage sensitive information with discretion and uphold client confidentiality
Preferred experience with managing program schedules (i.e., admissions/discharges); data extrapolation, MS Office, and Google Suite applications.

Travel -This position may require travel to the various state-run MHTL Homes and to in-person meetings and/or team building activities.
