Summary This Social Worker is assigned to the Community Housing and Outreach Services (CHOS) Programs at VA Puget Sound Health Care System located in Seattle, Washington with responsibility to provide direct services for Veterans, their families and caregivers. Responsibilities May travel to various sites within the catchment area, including Renton Annex, CRRC, American Lake campus, Seattle Medical Center, and Everett CBOC to provide support and oversight to staff members, as indicated. Data: Identify and monitor productivity goals for each CHOS program and for all CHOS staff members by utilizing relevant database systems and reports such as Mental Health and Social Work Profile, Homeless program dashboards and SharePoint sites, HOMES encounter reports, etc. Provide regular reports to the Program Director and/or Service Line Chief. Makes and implements recommendations to address areas of noted improvement, provide ongoing support to program staff and leadership to accomplish goals. Monitor CHOS program performance scorecard data and work with the Program Director, Program Mangers, and Program Supervisors to assist in the development of operational plans to address areas of improvement Supervise the SW Coordinator who performs monthly HOMES caseload reconciliation, quarterly Public Housing Reconciliations and associated action items including reconciling and updating the Project Based Registry. Addresses HUD-VASH database discrepancies, as reported by the HUD-VASH program office, and oversees compliance with the regular updates of the internal CHOS emergency management spreadsheet. Assist with notification and roll out of emergency management notifications to staff and Veterans. Updates internal spreadsheets for tracking of HUD-VASH voucher allotments and project-based site status. Clinical: Provide education to supervisors and staff on charting standards and expectations Assist in the development and ongoing updates of CHOS clinical documentation standards and templates. Audit staff electronic health records and provide feedback to staff and their supervisors on areas of needed improvement or non-compliance. Submit reports on compliance, trends, needed areas of re-training, other relevant findings to the Program Director. Develop processes for internal electronic health record audits in preparation for and in response to findings from Joint Commission and/or CARF surveys. Supervise the SW Coordinator responsible for the oversight of HUD-VASH contracted programs, ensuring clinical care is in accordance with the HUD-VASH federal regulations and providing support. Support the CHOS Director in the development and implementation of the Enhanced HUD-VASH Initiative, providing evidenced-based services and interventions across a multidisciplinary team to the geriatric population in HUD-VASH. Supervise the Grant and Per Diem and Contracted Residential Services programs and staff. Ensure for ongoing support of Liaisons' provision of services to Veterans residing in transitional housing. Administrative: Participate in the development of new partnerships and develop Memorandums of Understanding (MOU's), Service Agreements, and/or Revocable License agreements with partner agencies. Responsible for tracking, reviewing, and updating these documents in accordance with Office of General Council standards. Assist CHOS and the SW Service line in the preparation and ongoing readiness for Joint Commission and CARF surveys . Assist the Program Director in the development of the CHOS annual program budget and annual Needs/Excess budget update, including staff salaries, contracts, grants, clinical outreach (vehicles), and travel/training. Oversee the CHOS program SOP workgroup; ensure SOPs are reviewed annually and updated as needed. Assist the Program Director in developing and updating the annual operational plan. Assist the Program Director in analyzing outcomes of operational and programmatic goals, including performance measurement, performance improvement, stakeholder feedback, safety and patient related events. Make recommendation to the Program Director and implement plans to address areas of needed improvement. Supervise the Grant and Per Diem and Contracted Residential Services programs and staff, ensuring for appropriate, timely, and accurate billing, and effective fund control point management. Leadership: Assist the Program Director in the oversight of the CHOS program to ensure attainment of goals and compliance with National Directives. Provide support and education to Program Managers and Supervisors on programmatic changes, improvements, initiatives. Support the CHOS and SW Service Line in the development of SOP's, policies, presentations, etc., as requested. Participate in SW Service Line and Hospital-based committees, representing CHOS and SWS. Grade Determinations: GS-13 Supervisory Social Worker Experience/Education: One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as supervisory social workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, candidates must demonstrate all of the following KSAs: Ability to independently organize work, set priorities, and meet multiple deadlines. Skill in a range of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement. Ability to ensure provision of clinical social work services by supervised social workers. Ability to delegate authority, manage priorities, coordinate work, and follow up on pending issues to complete duties in an accurate and timely fashion. Ability to analyze organizational and operational problems, develop and implement solutions that result in efficient operations, and use data effectively to manage workload, quality, performance, and productivity within the service. Ability to provide staff training and development. Preferred Experience: Experience with homeless veterans and 3 years supervisory experience preferred.
