Summary The Social Worker (Program Coordinator) is a supervisory position for Department of Housing and Urban Development-VA Supportive Housing (HUD-VASH) located at H. John Heinz III Department of Veterans Affairs Medical Center in Pittsburgh, PA. The incumbent provides oversight of the direct clinical and case management services of veterans and/or families enrolled in the program. The program is primarily field-based and requires a valid driver's license. The incumbent is responsible for all performance measures for the HUD-VASH Program and reports monthly statistics to the National Office. They provide professional, clinical, and administrative management of their assigned HUD-VASH team in the Comprehensive Social Services Service Line under the umbrella of Healthcare for Homeless Veterans Program (HCHV) and community partners. HUD-VASH teams include multi-disciplinary staff, including nurses, social workers, and addiction specialist, who frequently travel between sites and spend approximately half of their time in the community. The HUD-VASH Social Worker (Program Coordinator) facilitates relationships both internally and externally to provide comprehensive services to a highly vulnerable veteran population. Internal coordination involves connections with inpatient medical and psychiatric services, HCHV Outreach Programs, Grant and Per Diem Programs, Veterans Justice Outreach, primary care clinics, outpatient mental health and substance abuse clinics, residential treatment programs and the CLC. Collateral partnerships include Supportive Services for Veterans & Families (SSVF), community shelters, local Continuum of Care organizations, public housing authorities, emergency personnel (Police, EMT/ambulance), municipal Veteran Service Officers, legal services, community nonprofits, community mental health and substance abuse providers, local Elder Services, and state agencies. As the supervisory program coordinator, the incumbent assists in planning, establishing, and implementing policies and procedures; developing individual or group goals and objectives as well as monitoring, operating, evaluating, coordinating, and overseeing the HUD-VASH staff to which they are assigned. They ensure that their staff comply with CARF, JCAHO and state licensure standards as well as VA-OIG expectations. The incumbent ensures that ongoing, intensive reviews and appraisals are completed to ensure the accomplishment of objectives and adherence to standards set by governing bodies. They collect and submit HUD-VASH Program participant data as outlined by HOMES evaluation procedures on a regular basis as well as in compliance with the VA, VHAPHS, VISN, VACO and HUD documentation and reporting requirements. The supervisor formulates and implements protocols and policies for the program on an on-going basis to meet the ever-changing needs of the program's target population. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: Experience/Education. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Skill in coordinating and implementing one or more specialty program(s), across the facility. Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. Knowledge of organizational structure and impact on program operations. Preferred Experience: Minimum two years prior supervisory duties and social work experience with homeless veterans referred.
