Summary The VISN 12 Clinical Resource Hub (CRH) Suicide Prevention (SP2.0) National Evidence Based Psychotherapy (EBP) Telehealth Program Licensed Clinical Social Work position incumbent provides a wide range of care including risk assessment and referral, intake/diagnostic and psychosocial assessments, comprehensive treatment planning, and evidence-based psychotherapy services in a manner consistent with established protocols to Veterans at risk for suicide. Incumbent thoroughly reviews the medical record and previous psychosocial assessments when the Veteran is referred, consulting with the referring provider as needed, and will complete an assessment of current symptoms/functioning/needs and provide a thorough treatment plan when a Veteran engages in CRH mental health services. Incumbent will provide evidence-based suicide prevention psychotherapies and interventions such as Cognitive Behavioral Therapy for Suicide Prevention, Advanced Safety Planning, Problem Solving Therapy for Suicide Prevention, and Dialectical Behavior Therapy. Incumbent will also provide crisis intervention within the @context of treatment. Major Duties include, but are not limited to: Maintain high skill and expertise to establish and maintain effective therapeutic relationships with Veterans and/or their families. Work with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems. Completes a current psychosocial assessment to determine veterans' psychosocial functioning and needs after thoroughly reviewing the medical record. Possesses a working knowledge and experience in the use of mental health and substance abuse diagnoses and treatment procedures, including acute, chronic, and traumatic illnesses, and suicide risk assessment and intervention. Participate as a member of a Suicide Prevention National Telehealth Program treatment team and engage in active collaboration with Veterans and their families as well as other interdisciplinary treatment team members, including, but not limited to, Psychiatrists, Nurse Practitioners, Psychologists, Social Workers, RNs, Peer Support Specialists, and Administrative Support staff, in the development and implementation of a treatment plan. Use of mental health and substance abuse diagnoses and treatment procedures, including acute, chronic, and traumatic illnesses, as well as in suicide risk assessment and intervention. Facilitate enrollment and discharge planning through collaboration with Veterans and their families, as well as interdisciplinary treatment team members, to ensure that appropriate discharge plans are executed in a timely manner. Address treatment compliance and "no-shows" when needed with patients. Communicate effectively, both orally and in writing, with people from diverse backgrounds. Provide emergency/crisis intervention services to outpatients experiencing acute psychological crises, help stabilize these individuals, assist in the referral arrangements for transfer to a psychiatric facility for inpatient care if necessary, and provide follow-up care. Provide case management services to Veterans and their families throughout the continuum of care. Utilizes principles of human growth and development over the life span and will be able to assist Veterans in coping with the loss and grief experienced from disability and terminal illness. Serves on committees and work groups at the facility as deemed appropriate by the supervisor. Using software applications for drafting documents and data management, as well as other computer systems used by the VHA. Perform additional duties as assigned. Education: Must have a Master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work. Licensure: Must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Requirements: In addition to the basic requirements stated above, the following qualification criteria must be met for the GS-12 Senior Social Worker position: Experience: Must have at least two years of experience post-advanced practice clinical licensure and should be in a specialized area of social work practice, of which one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-master's training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure: Must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, candidates must demonstrate all of the following KSAs: Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
